Director of Health and Wellness

Position: Health and Wellness Director
Reports to: Administrator/Executive Director
Position Overview:
The Health and Wellness Director at Magnolia Crossing Senior Assisted Living and Memory Care plays a pivotal role in enhancing the quality of life for our senior residents. This leadership position involves overseeing the Health and Wellness programs within the
community, ensuring residents receive compassionate, personalized care while adhering to the highest standards of safety and wellbeing.
Classification: Exempt
The following is a list of major duties and responsibilities for this position along with certain supportive duties. It is not all-inclusive.
Other duties and responsibilities may be added as needed and in addition, management, as appropriate, may modify this job
description.
Essential Duties and Responsibilities
 Leadership and Management:
o Provide visionary leadership to the Health and Wellness team, fostering a culture of empathy, collaboration, and excellence.
o Supervise, mentor, and evaluate medical technicians, caregivers, and other wellness professionals.
o Ensure that the Health and Wellness department operates efficiently and effectively.
 Care Planning and Implementation:
o Develop, implement, and review individualized care plans for residents, addressing their unique needs and preferences.
o Collaborate with residents, families, and healthcare providers to coordinate comprehensive care and support.
o Regularly assess residents' health conditions and adjust care plans as necessary.
 Medication Management:
o Oversee the administration of medications, ensuring safe and accurate delivery by nursing staff.
o Maintain accurate medication records and ensure compliance with state and federal regulations.
o Perform medication room and medication cart audits as needed.
 Resident Health and Safety:
o Review all physician’s transcriptions, progress notes, discharge orders, move in orders, and hospital notes.
o Promote a secure and nurturing living environment for residents.
o Establish and enforce infection control protocols and emergency preparedness procedures.
o Conduct routine safety inspections and address any concerns promptly.
 Wellness Programs:
o Develop and manage comprehensive wellness programs that encompass physical, mental, and emotional health.
o Collaborate with external healthcare providers for specialized wellness services.
o Oversee and participate in the development of care plans.
 Regulatory Compliance:
o Ensure full compliance with state and federal regulations related to senior assisted living and memory care.
o Investigate incidents and behavior changes that require a licensed nurse assessment and document all incidents and
changes in the resident’s progress notes.
o Maintain precise resident records and documentation, adhering to confidentiality standards.
 Budget Management:
o Assist in the creation and management of the Health and Wellness department budget.
o Monitor department expenses and identify opportunities for cost-effective practices.
 Resident and Family Relations:
o Foster positive and empathetic relationships with residents and their families, addressing their concerns and needs with
care.
o Uphold a culture of respect, dignity, and compassion within the community.
Management and Supervision:
 Ensure highly competent and committed staff in all areas of the facility. This includes recruiting, hiring, developing, training,
evaluating, counseling, disciplining, motivating and rewarding all employees.
 Ensure consistent application and administration of all Company policies and procedures, promoting open and honest
communications, and addressing all associate issues in a timely and fair manner.
 Contribute to the training and onboarding programs for care givers, medication technicians, and activity assistants ensuring
continuing education and staff development programs as needed based on State Licensing Requirements
 Conduct and/or participate in the Initial Orientation program for new staff members to ensure they have a firm understanding
of Company-specific policies.
 Maintain accurate record keeping, checklist assignments, and required documentation.
 Uphold professional codes of ethics, facility policies, and state/federal regulations.
 Provide support during afterhours telephone calls with staffing issues and emergencies.
 Participate in Manager on Duty (MOD) rotation to ensure appropriate coverage and management oversight.
 Participate in weekly Service Plan meetings, weekly Stand Up meetings, and weekly/monthly Management meetings as needed.
 Per State of California regulations, as a “mandated reporter”, you must report all known or suspected incidents of resident
abuse of dependent adults or elders. Any observed or known incidents should also be immediately reported to the
Administrator.
 Ensure a safe, healthy work environment for all staff, residents, and guests by following all safety, Health and environmental
policies, procedures and regulations per the Company’s IIPP, job safe practices and other related programs. This includes but is
not limited to participation in fire and safety procedures when needed.
 Ensure that a current listing of all oxygen tanks in the community is up to date including the accurate locations. The updated list
should be provided to the Health and Wellness Director, the Administrator, and the front desk for use in emergency situations.
 Perform any other duties assigned by the supervisor.

Request

Education and Experience:
 High school diploma or Bachelor's degree in Nursing, Healthcare Administration, or a related field preferred
 Maintains current state license as a Licensed Practical Nurse or Registered Nurse per state regulations.
 Minimum of 3-5 years of healthcare management experience, including at least 2 years in a senior assisted living and memory
care setting.
 Profound knowledge of regulatory and compliance requirements.
 Advanced skill in communicating orally and in writing with all departments
 Excellent leadership skills: inspires respect and trust, displays passion and optimism, motivates others to perform; gives
recognition and feedback, influences others effectively.
Other Qualifications:
 Minimum age of 18 years old.
 Successful completion of a Physical and TB Screening.
 Obtain Criminal Record Clearance.
 Ability to work in a team-oriented environment.
 Strong communication skills, both written and verbal.
 Proficiency in PC usage and Microsoft Office software.
 CPR and First Aid certification required.
 Valid driver's license for transportation coordination.
 Exceptional organizational, leadership, and interpersonal skills.
Working Conditions/Physical Requirements:
 This position involves both office-based tasks and active engagement with residents.
 May require standing, walking, and physical interaction with residents for extended periods.
 Flexibility in working hours, including weekends and evenings, to accommodate resident needs and activities.
 Occasional lifting of objects up to 50 pounds may be required.
 Exposure to various environmental factors such as noise, odors, and temperature variations.
 Some travel may be necessary for community outings and events.
