Center for Social Change is a private non-private organization that supports children and adults with Intellectual Disabilities, including Autism Spectrum Disorder. The Center is a strong advocate for personal choices in community living for individuals who are developmentally, physically, psychological and emotionally challenged .

Available Shifts

Monday-Wednesday: 11PM-9AM

Thursday-Sunday: 11PM-9/7/7/9AM

Qualifications:

  • Must be 21 years of age or older.
  • Possess and provide proof of high school diploma/ GED/ unofficial college or university transcript.
  • Must have a valid MD driver's license and acceptable driving record (less than 2 points).
  • Successful clearance of state and/or federal background checks.
  • Must have a Covid Vaccination

Certifications Preferred:

  • CPR
  • First Aid
  • CMT
  • Behavioral Management Technique
  • Mandated Training

Job Duties:

  • Staff makes sure that the home is in excellent condition at all times and meets the State required standards. Some of the specific responsibilities that surround maintaining the home will be: vacuuming carpets daily, cleaning floors and bathrooms, cleaning all appliances such as oven, microwave, toaster, dishwasher, refrigerator, washer and dryer, dusting windows, doors, furniture, television sets, blinds and curtains, organizing furniture in the house, and individuals' belongings such as their clothes, shoes, accessories etc., reporting repair work to property manager and program coordinator , picking up trash from inside home and outside home, reporting any damage to home and agency property including vehicles, careful use of gas, electricity, and water, and not bringing any personal items to the individual’s home.
  • Ensures individuals are fed on time with cooked food according to menu, that individuals are clean, groomed, dressed appropriate while at home and that they wear proper clothes that suit the season while outside on activities, appointments, parties, church or family visits; That grocery is purchased always with individuals of the home at the scheduled time every week and makes sure that sufficient groceries and supplies last for the week; That individuals' menu is updated every week; and that individuals have sufficient activities per current Activity Schedule; That special permission is sought if additional activities for individuals to take place for the day from On call administrator.
  • Ensure that individuals are supervised at all times for their safety; that all incidents are reported immediately to supervisor followed by an incident report, and follow strictly the Incident Reporting policy of the agency; That Individuals and all other staff including AWON, week-end staff and substitute staff meet weekly with individuals in a house meeting at a convenient time arranged by the house manager to discuss activities, concerns and minutes generated for that week.
  • Knowledgeable of all individuals' IP that includes their goals, short-term goals, interests and preferences, steps to achieve goals, how to implement the goals, individuals Behavior Plans, all related services that also include all medical services, individuals' behaviors, their diagnoses, medications, diet, special conditions, day programs, schools, service coordinators, case workers, family contacts and all emergency contact persons.
  • Thoroughly knowledgeable of Nursing Initial Evaluation and Initial Care plan followed by all 45 day reviews and care plans, individuals' weight change, diet change, medication change, and change in any services including medical services.
  • Conducts a three point check of individuals' medications, PMOF, and medication labels and assures other staff do the same to assure proper administration of medication to all individuals; That medication cabinet is always organized, locked at all times, discontinued medications removed, books arranged and medication policy and protocol are followed by all staff working in the home; That medication errors are reported to Delegating Nurse at all times; That all health-related issues are discussed with supervisor at all times, and reportable matters per medication administration manual reported to delegating RN in a timely manner.
  • Ensure that all paperwork related to all individuals and their home are done on time, and makes sure that other staff in the home do their paperwork on time; Such paperwork will include but not limited to Activity Schedule, Updated Menu per individual, IP goals and their data sheets, Behavior Plan and their data sheets, Daily documentation, Change of Shift form properly handed over and received at every shift, Fire and Disaster Drill, Seizure Records, Weight chart and all other charts, MARs, Incident reports, Individual Absence records, Leave Forms, Grocery receipts, Individuals' money receipts, Mileage log, and any other documents; That all required forms are requested from supervisor in writing, and makes sure that they are available at all times in the home for all staff, That paperwork are collected and placed in a bag so that they can be picked up by supervisor or on call administrator on a week, first of the month or when required.
  • Drive company assigned vehicle to include medical appointments for persons served and any other activities as directed by supervisor;
  • Ensure that the agency vehicle that is assigned to the home is maintained well per agency vehicle policy, and makes sure that the vehicle has gas in it, mileage log written by all staff, vehicle driven based on Activity schedule for individuals' activities or appointments; that agency vehicle is not misused by any staff working in the home. Misuse will include but not limited to personal use of the vehicle, rough use of the vehicle, eating and drinking, smoking inside the vehicle, not reporting damage to vehicle etc.
  • Ensure that safety measures in the home are taken always such as checking water temperature that should not be more than 110 at all times, making sure that other staff also check water temperature at their shift, monitoring individuals while bathing or showering, performing monthly fire drill or any other emergency drill for all individuals by following individualized evacuation plans and documenting the drill on the designated form, making sure that individualized evacuation plans are in place, and that all smoke detectors are in working condition, that there are no exposed wires, broken furniture, broken glass, doors, windows, light bulbs etc. in the home, making sure that restricted items such as knives, sharp objects, cleaning items etc. for some individuals are either kept away or locked per Behavior Plan or protocol, that door alarms and or window alarms for individuals are working, if applicable, that circuit breakers are off for individuals who have fire setting behavior, that body search is done and documented for those individuals whose IP require the staff to do, that individuals who smoke are always on eye sight supervision of the staff, that matches and lighters are not accessible to those individuals who are restricted from those, that individuals wear seat belts in the vehicle, that additional staff accompany driver for those individuals who have disruptive behavior in the vehicle, and that all other safety measures are in place as directed based on individuals changing behaviors per individuals' IP.
  • Ensure that the books and documentation on each individual is current and updated, IP book with IP and data sheets, Communication Folder with current data, OSHA Plan book that is read and signed off by all staff, Personnel Policy book, Free Time log for those individuals who use free time per IP, Daily individual documentation books, etc. are in place and that all current documents are filed properly in each folder, and also outdated documents are discarded from folders on time.
  • Ensure that the home has proper utensils, pots and pans, spoons and forks, knives, vacuum cleaner and its bags, trash cans with lids, trash liners, OSHA kits, light bulbs, curtains on windows, blinds on windows, flashlight, first aid kit, stationary items, batteries for flash light, detergents, mop for floor, toilet paper, hand soap and emergency kits.
  • Ensure that all individuals have seasonal clothing, and they wear only current seasonal and appropriate clothing, and that their previous seasonal clothing are stored in foot lockers; that individuals have personal hygiene items, thermometer, nail clipper, two pillows, two bed sheets, two bed covers, blankets, towels, wash clothes; That some individuals' helmet, walker, wheelchair, electric bed and any supportive devices for maintaining health and safety, are well maintained and are in place with doctor's order;
  • Familiar with individuals' day programs, day activities, medications in the day programs, contact persons, transportation contacts, lunch requirements, early dismissal, holidays and closure, bus timing, and incidents.
  • Ensures daily documentation of individuals' progress data, to administrators, attends major meetings of the individuals, and be part of individuals IP throughout the year.
  • Attends other required meetings, works overtime when asked, communicates with other staff on daily basis, oversees activities on weekend, and report deficiencies to supervisor.
  • Take initiative in organizing and participating in individuals' special functions such as Christmas, CSC anniversary, individuals' picnic, Halloween party etc.; that house manager arrange their individuals' birthday parties every year and lots of pictures taken, arrange home visits, vacations, and any other special functions.
  • Trained on all DDA/DHR mandated trainings, Medication Administration, and in some cases Certified Nursing Assistant, CPR/First Aid, and all specialized needs as required.
  • Ensure that individuals' medical appointments are attended when required, sometimes during morning hours when a day shift staff not scheduled to work.
  • Frequent lifting of up to 50 lbs may be required on a daily basis, depending on the specialized skills required per individual staff may have to utilize a hoyer lift, bath aid, or other means of lifting and transferring.
  • Any additional responsibilities as and when required besides the above mentioned.

  • This is an AWAKE OVERNIGHT position. You must be able to work OVERNIGHT without terms and conditions. You will be required to remain in the shift for at least 90 days.
