Job description

Job Summary:

Coordinates and participates on clinic's multidisciplinary diabetes team to improve delivery of services and quality of care; case management of patients with diabetes to promote optimal metabolic control, self-sufficiency, and to prevent complications of diabetes; and maintenance of a diabetes patient database.

Job Duties:

Routine duties shall include providing health care services to individuals eligible for services in accordance with the self-governance compact and funding agreement between the Cherokee Nation and the United States executed under the authority of the Indian Self-Determination and Education Assistance Act. Serves as chair or co-chair of clinic's diabetes team. Participates on clinic's multidisciplinary diabetes team to assess diabetes clinical practice, and to develop and implement policies and procedures to improve the quality of diabetes care. Initiates and participates in the establishment and operation of clinic's Diabetes Patient Education Program. Conducts intake assessment and evaluation of the newly-diagnosed patient and the new patient with diabetes to determine diabetes care and educational needs, and to facilitate patient's movement into appropriate care and services. Identifies patients at risk for diabetes complications. Supports patient in disease self-management and behavior change through culturally relevant interventions, including implementation of new treatment regimes, teaching, supportive counseling, problem solving, medication and status monitoring, and care planning. Coordinates and collaborates with other members of the diabetes team to achieve expected/desired patient outcomes. Documents case management and patient education activities in Patient Care Component using Subjective Objective Assessment Plan format and appropriate education codes. Creates and maintains database, including diabetes patient registry and supplies dispensed. Organizes, plans, implements, and evaluates the clinic's annual Indian Health Services diabetes chart audit, in conjunction with the Cherokee Nation Diabetes Program. Prepares and submits reports as required. Other duties may be assigned.

SUPERVISORY RESPONSIBILITIES

i.e. Lead position.

Qualifications:

EDUCATIONAL REQUIREMENT

Associate's degree from 2-year college or technical school in Nursing. BSN preferred.

EXPERIENCE REQUIREMENT

Two years nursing experience required, preferably ion diabetes patient care/education.

CERTIFICATES, LICENSES, REGISTRATIONS

Possess and maintain an Oklahoma Board of Nursing Approved License. Must be certified in CPR; Certified Diabetes Educator strongly preferred. Must possess a valid driver's license with a driving history verified through a motor vehicle report that meets requirements for Cherokee Nation underwriting rating.

OTHER SKILLS AND ABILITIES

Must have working knowledge of computers. Must be knowledgeable in Type 1, 2, and gestational diabetes. Must be knowledgeable of and be accountable for adhering to specialty practice standards of the American Nurses Association and the American Association of Diabetes Educators. Ability to speak the Cherokee Language preferred.

OTHER QUALIFICATIONS

Employee must not and will not be under sanction by the United States Department of Health and Human Services Office of the Inspector General (OIG) or by the General Services Administration (GSA) or listed on the OIGs Cumulative Sanction Report, or the GSAs List of Excluded Providers, or listed on the OIGs List of Excluded Individuals/Entities (LEIE).

Must meet and maintain pre-employment and periodic background investigation and adjudication for child care.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel and talk or hear. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision and ability to adjust focus.

WORK ENVIRONMENT

While performing the duties of this Job, the employee is occasionally exposed to fumes or airborne particles. The noise level in the work environment is usually moderate.
