Essential Duties and Responsibilities:
- Provides language interpretation to ensure efficient and accurate communication, ensuring patients understand care plans, medications, and follow-up.
- Assists with medical referrals and follows through on completion. Coordinates patient care with various pharmacies, hospitals, and other providers.
- Instructs patients on preparing for required procedures and tests. Accompanies patients to appointments. When necessary, advocates on behalf of high social- or medical-need patients.
- Accurately transmits messages between patients and care teams.
- Recruits, organizes, and serves as cultural navigator during group visits and workshops.
- Completes assigned language-associated tasks efficiently and expeditiously.
- Promotes patient independence.
- Appropriately documents activities in patients’ medical records.
Required Education and/or Experience:
- One year minimum work experience in community programs or in a healthcare setting.
Preferred Education and/or Experience:
- High school diploma, GED or equivalent.
- Medical Interpretation certification highly desired.
