Community Support Specialist - ADA
Department: Adult Behavioral Health
Job Status: Full-Time
FLSA Status: Non-Exempt
Reports To: AVP Adult BH
Grade/Level: Bachelor’s Degree
Amount of Travel Required: 50%

Position Summary
Provide direct and collateral services to outpatient adult clients in our Comprehensive Substance Treatment and Rehabilitation (CSTAR) program. Provide Community Support to clients and assist with making links /referrals to needed additional services.
Essential Functions Statement(s)
Provide direct and collateral community-based services to address activities of daily living including the management of symptoms associated with psychiatric, physical, and/or substance use disorders per program standards and the 8 Dimensions of Wellness.
Complete services consistent with the requirements outlined in the expectations worksheet and program manual with the supporting documentation completed in a timely manner (within 72 hours).
Maintain all assigned client medical records to program standards.
Provide referrals, advocacy, information and education to meet individual needs of clients.
Provide appropriate risk assessments and facilitate access to needed interventions as indicated.
Provide transportation for clients as needed.
Monitor client hospitalization and participate in discharge planning and follow per program standards.
Availability to provide services in evenings as assigned.
Attend court or other community based meetings as needed.
Maintain communication with referral sources and/or community stakeholders regarding client participation and progress in treatment, as needed.
Assist counselors with outreach to help ambivalent clients re-engage in treatment.
Assist with client UA's as needed.
Other clinical needs as necessary or assigned, including but not limited to, providing education to ADA clients in group setting.

Work Experience
Required Experience: 1 to 2 years
Experience: Prefer a minimum of one year experience working with inpatient and/or outpatient mental health or substance use disorder treatment.
Computer Skills: Basic computer skills required.
Certifications & Licenses: Must have valid driver’s license and auto insurance. Prefer work towards or maintain
substance use certification in Missouri. CPR certification preferred at hire, otherwise must be obtained within 30 days of employment, and maintained throughout
Other Requirements: Strong communication skills, written and verbal; Excellent analysis and problem solving skills; Able to work independently as well as function as a team member; Seeks/receives supervision
Education
Degree Level Required: Undergraduate
Bachelor's Degree (four year college or technical school) or equivalent experience Required, Field of Study: Social Services, Psychology, Health and Human Services, or related field preferred;
Equivalent Experience: meet qualifications as outlined by Department of Mental Health.
Work Environment
Health Center Clinics with regular visits to other homes and service providers within the community.
