The Community Support Specialist QMHP provides integrated, holistic, and Community Support services. They assist eligible children and their families to achieve or maintain optimum social, psychological, and physical functioning.
Essential functions include:
- Provide wraparound assessment for service planning
- Develop, implement, and monitor treatment plans and quarterly reviews
- Provide contact with families at minimum of twice per month based upon client/family needs, one at least face-to-face in the home. If the home is not a safe environment then at another identified location
- Maintain all assigned client medical records to program standards and complete documentation in a timely manner (within 72 hours)
- Provide referrals, advocacy, information and education to meet individual needs of clients
- Monitor families' progress and services they receive from various organizations
- Maintain communication between the family and service providers
- Assist families to identify and coordinate services
- Provide direct services to students
- Connect with the parents of students referred for services and provide information on other resources available
- Provide consultation, training, and support to school personnel
- Evaluate students in the program for needs, and assist the families in accessing resources to address these needs
- Act as liaison between the organization and the school district, the community, and/or the school district and the community
- Participate in the agency call rotation in order to provide emergency services to the community
Master's degree in psychology, social work, rehabilitation/vocational/pastoral counseling, family therapy or closely-related field required
- The completion of a practicum or at least one (1) year of experience under the supervision of a QMHP required
- All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
- A valid driver's license and agency established minimum automobile coverage required
