Role: Our Community Based Day Support Program is looking to hire a FT Community Specialist. This role takes members into the community daily. The employee must be comfortable driving a group of program participants in a company vehicle, and accessing the community with these individuals.
SUMMARY
Implement the mission of Community Support Program. This includes supervising members in the least restrictive community setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following (other duties may be assigned):
- Train assigned workers on community living and employment skills, i.e. money management (banking), use of community resources such as the library and restaurants, safety skills, shopping for work/interview clothes, job tours, volunteer work, personal grooming (barber or beauty shop) etc., in individualizing training methods to each person's learning style.
- Follow individual’s ISP goals and interventions, collect data as needed, and provide input into annual planning.
- Provide input in community schedule and suggestions for destinations.
- Teach and model appropriate social interaction, emphasizing appropriate habits, behaviors, and attitudes.
- Provide job coaching and training in community employment.
- Provide personal care as needed.
- Communicate effectively with Case Manager(s) regarding progress and performance.
- Respond appropriately to potentially problematic situations through direct intervention and/or obtaining support.
- Support the individuals to maximize community participation.
- Perform other duties as deemed necessary.
- EDUCATION:
- High School Diploma/GED required
EXPERIENCE: One year of experience in a related field
TRAINING: First Aid & CPR, Valid Driver License, CPI Cert or the ability to become certified. MAP certification a plus.
SKILLS / ABILITIES:
- Ability to communicate effectively in writing and verbally.
- Ability to interact effectively with disabled and non‑disabled people.
- Must have reliable transportation and drivers license to perform job
- MAP certification preferred
- Must be trustworthy, and capable of overseeing a small group independently in a community setting
