Job Summary:
Community Rehabilitation Specialist provides support to Adults in our Home and Community Based Services and CORE Programs with a focus on alleviating barriers to employment and education attainment in their community. The Community Rehabilitation Specialist will support the individual in achieving their social, interpersonal and employment and/or educational goals in a person-centered manner through refinement of skills such as emotional management, communication, self-care, socialization, personal adjustment, and relationship development.
Essential Job Duties:
- Form and maintain relationships with participants, community employers, and partners.
- Work with participant to identify Individual Service Plans (ISP) and develop goals consistent with their stated needs.
- Provide support to promote the development and growth of employability, both during job development and on the job.
- Assist with facilitating vocational and community habilitation workshops.
- Model professional work skills, including care of equipment, materials and property.
- Working knowledge of job market, post-secondary resources, community resources and public transportation.
- Written communication skills for documentation, including evaluations, progress toward individual goals, observations and interactions.
- Demonstrate competency with navigating electronic health record (EHR) for documentation of services rendered.
- Complies with all federal and state regulations and agency policies pertaining to HIPAA, false claims, whistleblower policies, and all others.
- Professionally represent our OLV Human Services in our community consistent with Agency Values.
- Keep storage, vehicles, and work areas clean and orderly.
- Any other duty as assigned by your Supervisor.

Minimum Education and Experience Required:
Bachelor's degree in Human Services, Education or related fields * Other BA/ BS disciplines will be considered contingent on experience
2 + Experience in behavioral health, SUD. Training, advocacy, education, or leadership is preferred.
Associates degree in Human Services, Education or related fields * Other AA/ AS disciplines will be considered contingent on experience
3 + Experience in behavioral health, SUD. Training, advocacy, education, or leadership is preferred.
High school diploma/ equivalent
4+ Experience in behavioral health, SUD. Training, advocacy, education, or leadership is preferred.
License/Registration/ Certifications Required:
Valid NYS Driver's License

- Trauma Informed Care/ RC
- Internal required training
- Funding-specific training: Essential Knowledge Areas & CORE Application of Principles (via CPI)
Physical Requirements:
