Form of workJoin our dynamic Adult Day Services team as a Licensed Practical Nurse (LPN), where you will play an integral role in our fast-paced program dedicated to supporting adults with developmental and intellectual disabilities. As a key member of our leadership team, you'll not only provide direct care but also serve as a mentor and guide to our dedicated Direct Support Professionals.
Your responsibilities will go beyond routine duties. You'll be instrumental in ensuring the health and well-being of our participants by overseeing and documenting their healthcare needs. Your swift response to emergencies will be crucial in maintaining a safe and supportive environment.

The Adult Day Services LPN will:
- Work consistently from 8:00 AM to 4:00 PM, Monday through Friday, ensuring coverage during program hours.
- Provide observation, monitoring and documentation for all Individuals we serve on presenting health needs.
- Administer medication as necessary.
- Perform medical rounds, nursing procedures and other treatments as required.
- Maintain accurate, neat, concise, timely documentation in all medical and agency records.
- Participate in Interdisciplinary and other health-related meetings as requested.
- Assist the Registered Nurse in addressing health teaching needs of the Individuals we serve, their families, and staff.
- Current NYS LPN Licensure (Required)
- High School Diploma/GED (Required)
- 2 years experience working with developmentally disabled population (Preferred)
