The Community Outreach Managerwill oversee governance and leadership of a Joint Venture Coalition that will work on a diverse range of projects and events with multi-disciplinary partner agencies with the goal of expanding COVID-19 vaccine access and supporting community resilience to vulnerable, under-served populations. This position will involve interaction with people from multiple organizations. Strong interpersonal and organization skills are required.

This is a full-time position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.

Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more.

Compensation: $65,000 - $75,000 annualized, depending on experience.

Responsibilities:

  • Coordinates with TVHC clinical staff and/or other TVHC departments for both community and health center patient education and engagement.
  • Supervise Community Health Outreach Health Workers (CHOWs).
  • Governance, oversight, and administration of the Coalition and working with four main community partners who make up the Coalition.
  • Facilitate on-going communication and scheduling between TVHC and all partners, conducting weekly and as-needed meetings to facilitate information sharing and planning.
  • Maintains documentation of program activities, and tracks data as related to TVHC patients, community members, and others.
  • Reports on grant deliverables as required by funders and contracts.
  • Develop and maintain a system of scheduling and referring patients/community members for health and recreational activities.
  • Utilize a collaborative problem-solving approach to resolve any issues as they may arise.
  • Work with all partners to coordinate all workshops, community activities/events, and other programs and services agreed to under each partners MOU and deliverables set.
  • Establish a data collection process with TVHC POD staff and Population Health/Data Department and all partners to collect daily, weekly, and monthly data on performance measures.
  • Provide cumulative Coalition daily reports at the close of business on the same day the PODs site operates on number of community members reached per zip code, number of community members who registered and received vaccine per zip code, methods of outreach/education and materials produced, number of bilingual or multilingual staff on site, number of referrals to the Language Line, and number of customer/client satisfaction survey collected.
  • Provide cumulative Coalition weekly and cumulative 30-day reports to Alameda County on performance measures.
  • Monitor program budgets to ensure partners and TVHC remain within budget.
  • Work with TVHC Finance Department to approve partner invoices and ensure partners are paid according to the agreed upon payment schedule.
  • Work with Marketing Department to develop outreach and education materials and an amplify communication campaign in coordination with all partners.
  • Continuously evaluate coalition effectiveness and coalition strategic plan.
  • Develop, organize, and maintain program documents.
  • Stay current with organizational strategies and program and department activities.
  • Perform all duties and services in full compliance with TVHC’s Service Excellence Standards.
  • Perform other duties as assigned by the Director of Partnerships and External Affairs, CEO, and other Executive Team members.

Requirements

  • High School Diploma (or G.E.D required)
  • Experience with grant and budgeting required.
  • BLS certification required.
  • AA or Bachelor’s degree in public health, communications, public health, health education or equivalent preferred.
  • Knowledge and understanding of complex health issues as they relate to the Latinx community and communities of color and their cultures.
  • Demonstrated ability working with Latinx, black, indigenous, and other people of color (BIPOC), and other oppressed groups.
  • Energetic, enthusiastic, resourceful, and committed to health promotion and healthy decisions.
  • Experience in Microsoft Office (Word, Excel, Outlook, SharePoint, Teams, etc.) required.
  • Demonstrated proficiency and knowledge of computer programs and general computer skills.

Qualifications:

  • Ability to build partnerships and relationships across diverse communities.
  • Experience with public speaking, events, and audience engagement both in-person and online
  • Bicultural, bilingual in English/Spanish preferred.
  • Excellent organizational and administrative skills.
  • Able to work effectively both independently and collaboratively. Self-starter.
  • Demonstrate flexibility regarding job duties and assignments.
  • Ability to travel to designated locations as directed by supervisor.
  • Ability to exercise good judgment and discretion when making programmatic decisions.
