The Community coordinator will provide education and screenings to South Asians at risk for developing Diabetes and its complications. The primary focus is on helping the community. This position will mainly be a work-from-home opportunity with attendance at outreach events as required. The ideal candidate should have an interest to engage with the community.
Responsibilities:
- Create an annual outreach plan to educate the community about health disparities in South Asians, especially metabolic disorders
- Develop a plan to recruit, train, and utilize volunteers for community based screenings.
- Attend relevant community meetings and participate in relevant committees.
- Schedule presentations in the community and help build alliance with other stakeholders
- Provide resources and referrals to local and community-based services to patients to help navigate improved health outcomes
- Recruit community members and families for participation in the various SKN programs
- Be knowledgeable about the cultural practices and needs of the population being served
- Compile annual report of summary of services, including demographics of cohort, support services rendered, challenges, and results of program
- Capacity building in basic advocacy skills
- Perform additional duties as required
