Job Description The Caring Transitions Team is comprised of Social Workers, Peer and Family Advocates, a Transitions Coordinator, and a Medical Director, who will support children, youth, and their families who present at H+H hospitals in Queens following a suicide attempt or serious suicidal behavior, from point of entry until 90 days post-discharge.
The Caring Transitions Team will engage with youth and families upon presentation to the ED or direct admission. If interested in participating, the team will follow them throughout the hospital course, participating in aftercare planning. Upon discharge the team ensure that youth are connected to outpatient treatment, provide caregivers with tools to understand and identify risk factors and warning signs of suicide, assist the youth in developing life-affirming goals, and develop the capacity of other trusted adults and community supports in the lives of the youth and families they serve.
In certain cases, the team will provide bridging care/treatment until the youth is connected to outpatient services. Duties & Responsibilities The Family Advocate is an integral part of this interdisciplinary team. Under supervision, the candidate for the position uses their lived experience navigating one or more systems that serve children and families.
They are a parent or primary caregiver of a child or young person with significant social, emotional, developmental, medical, substance use, behavioral health, and/or suicide attempts. The Family Advocate provides direct services to families in emergency, inpatient, outpatient, home and specialized settings. Works in close collaboration with staff, consumers and family in planning and implementing treatment plans, supporting and navigating the system of care.
AREAS OF RESPONSIBILITIES: Parent support, advocacy, system linkage, access to care. 1. Engages patients, parents and/or caretakers and/or community providers, as applicable, and provides resources and assistance needed to address issues preventing them from staying healthy 2.
Maintains required documentation/tracking in electronic medical record (Epic) and use other electronic systems in accordance with established policies and procedures. 3. Supporting families to develop formal and informal networks of support 4.
Helping families understand mental health diagnoses, medication, and treatment options 5. Engaging Families in service 6. Working with families to discern their strengths, needs, and priorities 7.
Providing information and referral 8. Providing 1:1 assistance to help families connect to and participate in services 9. Increasing parents' feelings of self-efficacy 10.
Reducing stigma and feelings of isolation 11. Helping families identify goals and track progress 12. Empowering parents by increasing their knowledge and skills 13.
Initiating and supporting parent support and skills groups 14. Helping families learn about services 15. Outreaches families post-visit 16.
Conducts community outreach 17. Assists in the preparation of information and educational materials 18. Report regularly to Supervisor for support and feedback; participate in program improvement efforts 19.
Other duties as assigned Minimum Qualifications For Appointment to Level I 1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or 2. Completion of an approved six month training program of combined classroom training and on-thejob experience in Community Liaison work, plus six months of full-time experience as described in "1" above.
In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires: 1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or 2. Education and/or experience equivalent to "1" above.
Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the fulltime experience described above. Department Preferences New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.
