We are looking for a Liaison to join our team and serve as a bridge between our organization and external stakeholders. The successful candidate will be responsible for facilitating communication between the two parties, ensuring that information is exchanged accurately and in a timely manner. The Liaison will also be responsible for coordinating meetings, preparing reports, and providing support to the organization’s internal departments. Create community partnerships/maintain relationships with Assisted Living facilities and Independent Living facilities.
This position will be responsible for generating hospice referrals from community partners.
Responsibilities:
- Establish and maintain relationships with key stakeholders.
- Understand the needs of the organization and its stakeholders.
- Gather and analyze data to identify trends and patterns.
- Develop and implement strategies to improve stakeholder engagement.
- Facilitate communication between stakeholders and the organization.
- Provide feedback to stakeholders on their progress towards achieving goals.
- Organize meetings, workshops, and other events to promote collaboration.
- Generate referrals from community partners.
- Market continuum-of-care to community partners.
Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
