Shift Details: This position is budgeted for 24-40 hrs/wk for 1 year grant with FLEXIBLE HOURS

• This is a grant-funded position budgeted for 9-12 months of service. This is a fantastic Per Diem opportunity for someone looking to accrue hours towards their Community Health Worker (CHW) licensure!

Project Overview:

For this project, incumbent will be part of a team that serves patients with diabetes who live in Bridgeport and consent to participating in this project. Interventions include access to healthy foods, enhanced registered dietician time for nutritional, exercise and culinary education, enhanced Community Health Worker time, and motivational interviewing/education and training.

The Community Health Workers will:

• Connect patients and families to resources and services that address their social influencers of health.
• Provide motivational interviewing and social assessments to help set goals and evaluate progress.
• Provide referrals and connection to service.
• Provide other assistance as necessary.

This is a fixed duration Grant Funded Position.

Qualifications

Education:

• High School diploma or equivalent.

Licensure:

• In process of working towards the CT Community Health Worker certification required.
• CT CHW Certification preferred

Experience:

• 100 hours of CHW worker experience preferred but not required.
