Community Health Worker

POSITION SUMMARY

Under supervision, the Community Health Worker (CHW) will implement educational campaigns and community outreach to promote vaccine confidence and address vaccine hesitancy within the greater Hartford Community with a focus on high-risk, underserved populations with low COVID-19 vaccine uptake. The Community Health Worker will engage, assess, identify, and link qualifying individuals to HHS for COVID-19 vaccine and/or Testing, Flu vaccine, and other services at HHS including and not limited to health coverage access, primary care, and other related services within the community. The CHW will work with community partners on project-related outreach, canvassing, tabling events, door-to-door outreach, and recruitment activities, and will implement community-based education sessions in person/or virtual. Maintains detailed outreach data to support programmatic reports. This includes providing culturally and linguistically appropriate assistance to families to meet their basic needs and increasing Vaccine Confidence as a trusted messenger.

This is a grant funded position. The position is contingent on grant funding.

RESPONSIBILITIES

• Promote and provide educational sessions on vaccine confidence and safety, access, and HHS services
• Implement and collaborate on community activities through a variety of mechanisms including in-person and online platforms.
• Conduct outreach to relevant community partners through collaborative activities and attendance at community and stakeholder events, health fairs, PTO, and school engagement activities;
• Conduct outreach to engage, educate, and recruit community participants in community and nontraditional settings (mobile vans, out-patient centers, walk-in clinics, barber shops, and other venues)
• Connect and follow-up with participants to community resources so their needs can be met related to improving health outcomes i.e.; PCP linkage, Health Insurance
• Establish and maintain positive relationships with potential and enrolled participants, visitors, and other employees;
• Work flexible hours (may include some evenings, nights & weekends) to accommodate the needs of the community, as needed;
• Maintain professional behavior in non-traditional work settings (public venues, informal gatherings, outdoor events, etc.);
• Work independently, exercise initiative, and use discretion when handling confidential information;
• Contribute to the project's outreach and retention strategies;
• Track and document outreach and engagement activities as required by grant;
• Regularly provide updates on the progress of project activities to ensure appropriate and timely enrollments, quickly identify difficulties, and implement corrective measures;
• Attend project's specific and related trainings and meetings as requested by the project's leadership;
• Conduct all project activities by standard operating procedures and other appropriate agency's regulations, procedures and policies, including procedures to safeguard the confidentiality of participants; and
• Perform other duties as assigned.

EXPERIENCE, SKILLS & MINIMUM REQUIRED QUALIFICATIONS

• High School diploma or GED equivalent and at least 3 years of community-based outreach experience and health promotion
• Must possess cultural awareness, sensitivity, and humility with a familiarity of the social determinants of health impacting the greater Hartford community
• Excellent interpersonal, communication and organizational skills
• Ability to manage multiple priorities under pressure
• Ability to plan and implement program initiatives
• Ability to maintain professional boundaries in diverse public settings working both independently and within a team
• Professional or personal experience supporting individuals with challenging needs
• Proficiency in computer and digital applications (Microsoft, Excel, Teams, Canva, Social Media Platforms)
• Basic knowledge of local population demographics, assets, and needs
• Must have a valid in-state driver's license
• Strong communication and organizational skills

EXPERIENCE, SKILLS & PREFERRED QUALIFICATIONS

• CT State Certified CHW highly preferred or Associate in Human Service, Social Service or Public Health related field
• Experience in conducting outreach community engagement and education
• Bilingual (Spanish, Portuguese, Haitian Creole, Arabic and Karen)
