*This is a grant-funded position budgeted for 9-12 months of service. This is a fantastic Per Diem opportunity for someone looking to accrue hours towards their Community Health Worker (CHW) licensure!

Project Overview:
For this project, incumbent will be responsible for work related to two grants. There will be a focus on Breast Health, encouraging community members to receive screening mammography and promoting breast health. In addition, incumbent will focus on identifying health-related social needs and work to meet those needs through available resources and community connections. The incumbent will serve as part of the Mission Services/Community Health team and will be involved in many Community Health related programs.
The Community Health Worker will:
- Connect patients/community members and families to resources and services that address their social influencers of health.
- Provide referrals and connection to service.
- Participate in bi-weekly food distribution.
- Attend health fairs and other community-based health promotion events.
- Participate in Medical Mission program.
- Assist with CHW caseload as needed to ensure continuity of care.
- Ongoing data entry tracking client progress and interactions.
This is a fixed duration Grant Funded Position.

Qualifications
Education:
- High School diploma or equivalent.
Licensure:
- In process of working towards the CT Community Health Worker certification required.
- CT CHW Certification preferred
Experience:
- 100 hours of CHW worker experience preferred but not required.
