Our top-rated organization is looking to add a full time Community Health Worker-HIV PrEP Navigator to assist with a new project in the Division of Infectious Diseases (HIV Pre-Exposure Prophylaxis) at UConn Health located in Farmington, CT.
At UCHC, this class is accountable for performing a full-range of tasks to assist in the assessment, evaluation, and treatment planning for clients in community programs, as well as providing ongoing community support services to program/project clients.
The ideal candidate for this position of a Pre-Exposure Prophylaxis (PrEP) Patient Navigator/Health Educator will be a highly motivated and compassionate individual who will play a crucial role in supporting individuals at risk for HIV and who have been referred for PrEP Navigation services. This position provides support to providers who prescribe and manage PrEP by monitoring, tracking, and sending reminders regarding laboratory testing, prescription refills, and follow-up appointments. The PrEP navigator works closely with patients taking PrEP for HIV prevention to assist with adherence to treatment and retention in care. This position will also provide in-depth education to patients regarding the risk of HIV exposure and infection, safe sex practices, and importance of persistence in PrEP care for HIV prevention. The PrEP Patient Navigator/Health Educator will support improvements in health outcomes through coordinating care, educating patients, building trust between patients and medical providers, and by enhancing communication and continuity of care. In addition, this position will track linkage, treatment and referral outcomes by maintining patient and program records, records of the services provided and timely completion of all required documentation (intakes, appointment reminders, appointment follow-up, referral, etc.).
SUPERVISION RECEIVED:
Works under the general supervision of an employee of higher grade (MD and/or RN) in the performance of routine tasks. Receives closer supervision on more advanced complex issues.
EXAMPLES OF DUTIES:
  • With the assistance of the program supervisor, performs an assessment of client's strengths and needs.
  • Works as part of the multidisciplinary team to prepare and implement plans.
  • Educates and counsels clients
  • Assists clients in linking with community support systems and agencies, monitors client progress within the health/community support systems.
  • Provides education and training to clients in community living, activities of daily living skills.
  • Provides client outreach.
  • Completes all required documentation.
  • If specified, serves as a translator for the staff and parents.
  • Performs related duties as required.
MINIMUM QUALIFICATIONS REQUIRED
KNOWLEDGE, SKILL, AND ABILITY:
  • Knowledge of community resources or providing supplementary assistance (as specified, e.g. HIV program, Ryan White programs, AIDS Service organizations and other public support systems) to clients/families.
  • Oral and written communication skills.
  • Considerable interpersonal skills.
  • Ability to work with a diverse client population.
  • Ability to read/write English, and fluency in Spanish is not required but would be of benefit.
EXPERIENCE AND TRAINING:
General Experience:
Four (4) years' experience in the community working with human services or social services and recovery assistance programs.
SUBSTITUTION ALLOWED:
College education in human services, social services, peer support or recovery coach field may substitute for the General Experience based on fifteen (15) semester hours equaling one-half (1/2) year of
experience to a maximum of four (4) years for a bachelor's degree.
SPECIAL REQUIREMENTS:
  • Must have a valid driver's license and access to reliable transportation.
  • The position may require travel to multiple sites across the state.
  • Experience working with diverse populations.
PREFERRED REQUIREMENTS:
  • Experience in providing education and care coordination to HIV negative individuals who require assistance in accessing Pre-Exposure Prphylaxis (PrEP) prevention services.
  • Experience completing confidential assessments to determine patient needs, regardless of HIV status.
  • Experience providing treatment adherence counseling to ensure readiness for a medication regimen.
  • Experience providing routine telephone outreach to patients.
  • Experience working with providers in the coordination of services.
  • Experience tracking linkage, treatment and referral outcomes.
  • Experience maintaining accurate records in accordance with applicable standards, regulations, and requirements.
  • Experience utilizing an Electronic Medical Record (EMR) to document patient services.
  • Excellent oral and written communication skills, as well as interpersonal skills and ability to work as a part of a team.
  • Excellent organizational skills.
  • Ability to read, write and speak in English and Spanish.
  • Experience using Microsoft Office.
WORKING CONDITIONS:
Incumbent in this class may be exposed to communicable diseases.
SCHEDULE: Full time, 40 hours per week, generally Monday-Friday, 8:00 am to 4:30 pm, with a 30-minute unpaid meal break. Flexibility is required to meet department needs as well as to meet the needs of our community partners whose operating hours vary.
