Coordinates and supervises the personnel and function of the ambulance service Community Paramedicine Program and staff assigned to it. Responsible for the administrative and operational activities of the program to facilitate maximum accuracy, cost efficiency and productivity. Generates strategies for improving the Community Paramedicine Program operations through active participation in the continuous quality improvement philosophies.

ESSENTIAL FUNCTIONS:
- Establishes relationships with other areas of the hospital who refer patients to the program, including CHF Navigator, Iowa Heart Nurse Practitioners, Physicians, Care Coordinators and Home Health Care Agencies.
- Manages the entire case load of patients currently enrolled in the Community Paramedicine program.
- Assigns case load to ambulance staff members assigned to the Community Paramedicine program.
- Maintains a case load of patients assigned to them in the Community Paramedicine program.
- Provides in home visits to patients that include medical, safety, behavioral, and nutritional assessments.
- Refers patients to community-based programs based on their Social Determinates of Health.
- Provides IV medications, monitoring of vital signs, weight, blood clotting factors, and blood glucose levels as required by patient’s physician and patient care team.
- Participates in patient care team meetings for each patient helping prepare the Care Plan for them.
- Review and quality check work of staff working in the program
- Provides follow up reports to Clinical staff involved in enrolled patient’s care.
- Maintains a high standard of courtesy and cooperation in dealing with peers, clients and colleagues.
- Monitors and maintains work areas to provide efficient use of resources and obtain optimal turn around times.
- Assists in the formulation and implementation of department goals.
- Works with staff to resolve and identify opportunities for process improvement. Facilitates process review and implements identified changes.
- Ensures compliance with regulatory and accrediting agency standards.
- Performs other responsibilities as requested by the Manager, Ambulance and the Director of Emergency Services.
MARGINAL FUNCTIONS:
- Serves on hospital or departmental committees as assigned by the Manager or Director.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Current certification as a Critical Care Paramedic in the State of Iowa required.
- High school diploma preferred or high school equivalency.
- A minimum of five years of Paramedic experience required.
- One year of prior supervisory experience preferred.
- Basic Life Support (BLS) certified or obtained within three (3) months of hire. The acceptable credentialing body for this requirement is the American Heart Association.
- Advanced Cardiac Life Support (ACLS) certified or obtained within three (3) months of hire. The acceptable credentialing body for this requirement is the American Heart Association.
- Proof of completion of Dependent Adult and Child Abuse Mandatory Reporter training within three (3) months of hire.
- Valid Driver's License required, must meet Mercy’s Motor Vehicle Safety Standards, must be at least 18 years of age and be eligible to drive per Iowa state law.
