Job description

Overview:
Provides care coordination for all cancer sites across the cancer care continuum, assessing the physical, psychological and social needs of the patient and family. Establishes and maintains relationships with medical providers and provider organizations to offer information, education and coordination of patient care across the continuum. Leads coordination efforts to assure appropriate support staff are utilized through transitions across the continuum. Assists in implementing identified plan of care with desired outcomes. Serves as an ongoing resource for patients during diagnosis and through survivorship as necessary. Completes assessment, planning, implementation and evaluation of programs as assigned.

Responsibilities:
Coordination of Care
  • Coordinates interdisciplinary care of patients with a potential or actual cancer diagnosis, all disease sites, by working closely with providers, Cancer Committee and Director to develop site specific programs and services.
  • Coordinates patient care by serving as the point-of-contact for ongoing resource for patients and families throughout the care continuum - pre-diagnosis through treatment into survivorship.
  • Follows patients through the care continuum/experience, eliminating operational barriers (such as scheduling, test results, transportation, financial, cultural, language, etc) to service.
  • Communicates information regarding patient status and needs to providers and identified care team members including but not limited to: care/case managers, social workers, nurses, dietitians, mid-levels, pathology and/or palliative care. Recommends support services consistent with the patient’s unique needs.
  • Facilitates scheduling of appointments with specialists to assure coordination of care and timely treatment.
  • Identifies developmental, physical and psychological needs to provide age appropriate care and services to meet needs of patient and their families.
  • Demonstrates expertise in education of patient/family along all aspects of the continuum. Collaborates with other care team members in developing, implementing and evaluating education materials for patients and their families. Routinely updates patient/family education materials. Provides referrals for appropriate support groups and services. Assists with coordination of support groups for tumor specific programs, as requested.
  • Assumes role of clinic nurse, assisting with multidisciplinary clinics and patient procedures in HGNCCC, Breast and Bone Health and STL Radiation Center adhering to department specific policies and procedures, as requested.
Programmatic Initiatives
  • Works in collaboration with cancer care leadership to develop and monitor clinical quality indicators that reflect the entire continuum of care. Assists with development of action plan for indicators that fall below established benchmarks.
  • Ensures accurate and timely data collection and entry into patient database. Maintains patient tracking database.
  • Performing insurance pre-authorizations for medications, diagnostic tests, and surgeries
  • Assists with preparation and presentation of multidisciplinary care conferences ensuring communication with primary physician, oncology specialists, surgical specialists, radiology, pathology, etc. Takes reports to conference each week in order to facilitate proper communication about each presented patient. Documents discussion of evidence based national guidelines and clinical stage, as requested.
Qualifications:
  • Graduate of accredited nursing program
  • Comprehensive knowledge of current clinical and specialized patient care concepts related to own professional area.
  • Current licensure and in good standing to practice as a Registered Nurse in Iowa.
  • Must possess and maintain current Basic Life Support (BLS) certification
  • Ability to work independently and self-direct work activities.
  • Critical thinking skills using independent judgment in decision making.
  • Detail oriented and able to meet established deadlines.
  • Writes, reads, comprehends and speak fluent English.
  • Multicultural sensitivity
  • Microsoft Office-basic computer skills.
  • Flexibility in schedule to meet patient/clinic needs.
