Community Habilitation Specialist

We are looking for experienced Community Habilitation Specialists who enjoy working with people in need. Our ComHabs are responsible for assisting with daily living skills, socialization, community outings, medication administration and many other tasks.

Female Specialists

• Brooklyn 11229: 10 year old male who has 15 hours a week of respite to enrich his social and commination skills with recreational activities, sat & sun 4 pm - 10 pm Diagnosis: intellectual disability and autism
• Brooklyn 11228: 46 year old female, who has 9.5 hours a week of Community Habilitation to work on independent living skills goal. Monday's & Tuesday's 9 am - 12 pm and Sunday's open availability; Diagnosis: intellectual disability and epilepsy
• Brooklyn 11204: 17 year old male who has 6 hours a week of respite, sat & sun 2 pm - 5 pm Diagnosis: intellectual disability and autism
• Brooklyn 11239: 28 year old female, who has 9.5 hours a week of Community Habilitation to work on independent living skills goal. weekend availability Saturday & Sunday morning after 11am; Diagnosis: intellectual disability and autism. Diagnosis: intellectual disability and autism
• Brooklyn 11201: 23 year old male, who has 6 hours a week of Community Habilitation to work on independent living skills goal. weekend availability Saturday & Sunday 1pm-4pm; Diagnosis: intellectual disability and autism. Diagnosis: intellectual disability and autism
• Brooklyn11207: 22 year old male who has 10 hours a week of Community Habilitation to work on independent living skills goal and street safety skills as well as money management. Any 2/3 days of te week after 12 pm . Diagnosis: intellectual disability and autism

• Brooklyn 11203: 16 year old male who has 9.5 hours a week of respite for recreational activates, family has open schedule anything after 4:30 pm. Diagnosis: intellectual disability and autism
• Brooklyn 11233: 60 year old male who has 12 hours a week of Community Habilitation to work on independent living skills goal. Monday's & Friday's starting after 4:30. Diagnosis: intellectual disability and autism

Yoruba speaker female

• Brooklyn 11238: 54 year old female who has 9.5 hours a week of respite and 9.5 hours a week of CH. Saturday & Sunday starting 9 am- 3 pm, JW church and must go on Sundays Diagnosis: intellectual disability, epilepsy and disorder of central nervous system

Mandarin speaker female

• Brooklyn 11214: 13 year old male who has 12 hours a week of Community Habilitation to work on independent living skills goal. Tuesday, wed Friday after 3 pm & sat 12 pm; Diagnosis: autism
• Brooklyn 11355: 11 year old female who has 16 hours a week of Community Habilitation to work on independent living skills goal. Tuesday, Thursday and Fridays 4pm- 8pm ;Diagnosis: intellectual disability and autism

Bilingual Spanish female/male-

• Brooklyn 11237: 18 year old male who has 9.5 hours a week of Community Habilitation to work on independent living skills goal. Tues & Fridays after 5 pm - 8 pm and Saturday's 12:00 pm - 3:30 pm Diagnosis: intellectual disability and autism

Bilingual Spanish male

• Brooklyn 11214: 14 year old male who has 9.5 hours a week of respite for recreational activates and 7.5 of Community Habilitation. , family has open schedule anything after 4:30 pm. Diagnosis: intellectual disability, autism and down syndrome

What is the WORK EXPERIENCE requirements?
The ideal candidate will have a background in education or healthcare, and have excellent communication skills and the desire to help others.

What are the RESPONSIBILITIES?

• Perform assessments on residents.
• Teach residents living skills.
• Communicate with staff to coordinate care plans.
• Make sure that residents are safe and well cared for.
• Supervise residents when they are engaging in activities outside of their room.
• Create daily schedules for group activities.
• Be actively friendly to residents, family members, and coworkers.

What is the EDUCATION requirements?

• High School Diploma or equivalent required (we provide training)
