Community Habilitation Specialist

We are looking for an experienced Community Habilitation Specialist who enjoys working with people in need. Our ComHab Specialist are responsible for assisting with daily living skills, socialization, community outings, medication administration and many other tasks.

 Bronx 10465: Verbal 9 year old male, 5 hours a week of Community Habilitation to improve independent living skills. Desired schedule: Friday's after 3:30 pm  . Diagnosis: Autism

 Bronx 10468: verbal 22 year old male, 10 hours a week of respite to partake in social recreational activities and improve his money management skills, Monday, Wednesday and Friday. Diagnosis: Intellectual Disability and autism
 Bronx 10468: verbal 14 year old female, 10 hours a week of respite to partake in social recreational activities and 2 hours a week of CH improve his money management skills, mon-thurs Diagnosis: Intellectual Disability and autism
 Bronx 10453: Verbal 17 year old female, 15 hours a week of Community Habilitation to improve independent living skills. Desired schedule: open schedule any day of the week. Diagnosis: autism Intellectual disability
 Bronx 10453: verbal 15 year old male, 12 hours a week of respite to improve social and recreational activities, anytime sat & sub. Diagnosis: Intellectual Disability and autism
 Bronx 1045: Verbal 28 year old female, 10 hours a week of Community Habilitation to improve independent living skills and assist her with enrolling into college and travel training. Desired schedule: Monday-Wednesday after 12 pm Diagnosis: Intellectual disability
 Bronx 10467: verbal 9 year old male, 7 hours a week of respite to partake in social recreational activities, 10 am - 1:30 pm sat & sun. Diagnosis: Intellectual Disability
 Bronx 10462: verbal 16 year old male, 16 hours a week of respite to partake in social recreational activities, flexible schedule, after 4 pm any day during the week. Diagnosis: Autism
 Bronx 10455: Verbal 12 year old male, 15 hours a week of respite to partake in social recreational activities and 10 hours a week of Community Habilitation goal oriented. Desired schedule: fri after 4 pm and sat & Sunday flexible schedule after 11 am. Diagnosis: Intellectual disability, emotional outbursts
 Bronx 10455:  verbal 14 year old male, 2 hours a week of Community Habilitation to improve independent living skills and 23 hours a week of respite for recreational activities. Desired schedule: mon,wed&fri after 3 p:45 pm and open weekends Diagnosis: autism
 Bronx 10451:  verbal 1 year old female, 10 hours a week of Community Habilitation to improve independent living skills and 16.5 hours a week of respite for recreational activities. Desired schedule: mon,wed & Friday 5 pm - 8 pm and Saturday 12 pm-4 pm. Diagnosis: autism
 Bronx 10468: Verbal 18 year old male, 10 hours a week of respite to partake in social recreational activities and 10 hours a week of Community Habilitation goal oriented. Desired schedule: Monday-Friday 4-7. And sat 12-5pm Diagnosis: Intellectual disability, emotional outbursts

 Bronx 10457: Verbal 17 year old male, 4 hours a week of Community Habilitation to improve independent living skills. Desired schedule: sat or sun after 11am . Diagnosis: Autism, emotional outbursts

 Bronx 10457: Verbal 24 year old male, 10 hours a week of Community Habilitation to improve independent living skills. Desired schedule: Sat and Sunday after 1 pm -5. Diagnosis: Intellectual disability

What is the WORK EXPERIENCE requirements? 
The ideal candidate will have a background in education or healthcare, and have excellent communication skills and the desire to help others.

What are the RESPONSIBILITIES?

• Perform assessments on residents.
• Teach residents living skills.
• Communicate with staff to coordinate care plans.
• Make sure that residents are safe and well cared for.
• Supervise residents when they are engaging in activities outside of their room.
• Create daily schedules for group activities.
• Be actively friendly to residents, family members, and coworkers.

What is the EDUCATION requirements? 

• High School Diploma or equivalent required (we provide training)
