Job Description
- Provide initial and ongoing training and eventually oversee the development and implementation of Person Centered Support Plans for each Core Member to include monitoring goal progression.
- Conduct regular leadership meetings to include Community Home Supervisors, office staff, etc. including case managers and family members, for example, as needed.
- Work collaboratively and participate in community-level decisions making with Community Home Supervisors, Community Coordinators and other community leadership in problem solving, information sharing, and team building to support the changing needs of the community as a whole.
- Support Community Home Supervisors in providing appropriate and effective services for Core Members including scheduling appointments and follow up care, coordinating monthly pharmacy needs, and engaging community nurse as appropriate.
- Oversee and provide guidance regarding compliance with all county, state, and L’Arche regulations, policies and procedures.
- Collaborate with Community Home Supervisors regarding Direct Support Professionals scheduling to ensure coverage needs for the home are met.
- Collaborate with Community Home Supervisors to provide CPR/AED/FIRST AID and annual state trainings for all employees.
- Complete Personal Files
- On call duties for the community will include approximately one (1) week per month.
- Audit record keeping for each Core Member including but not limited to risk assessment, behavioral support plan, medical and financial information and annual plan documentation.
- Provide guidance and support to Community Home Supervisor as needed to ensure household finance and spending are organized and in line with designated budget.
- Provide guidance and support to Community Home Supervisors to ensure upkeep and maintenance of home and other assets. Input new hires on ADP

- Computer literacy with Microsoft Office
- Associate’s or Bachelor’s degree in human services or a related field (highly preferred)
- High school diploma or GED required.
- Some supervisory or management experience required.
- At least two (2) years experience in a residential, group home type setting (highly preferred).
- Periods sitting at a desk and working on a computer.
- Must be able to perform activities of daily living such as cooking and cleaning.
- Able and willing to assist in house maintenance/yard work.
