Salary: $59,482.50

Summary: The Program Coordinator of Community Living Services is responsible for completing clinical, programming and administrative functions to ensure quality, compliance and person centeredness throughout the assigned caseload. This position also maintains close and positive relationships with referral/sponsoring agencies and regulatory bodies.

Reports To: Director of Residential Services

Supervises (Position Title): Residential Home Manager

Principle Duties / Responsibilities:

· Provides direct supervision, coaching, feedback and evaluation to Residential Managers with the completion of his/her job duties.

· Assist the Director of Residential Services with preparing payroll.

· Ensures apartments and homes are maintained in proper condition to ensure compliance to regulatory requirements and support for the life-style of people served.

· Works with the Client and their team to develop the Person-Centered Individual Service Plan (ISP) process, planning and implementation is completed.

· Ensure and monitors residents progress toward achieving objectives at least monthly and adjusts implementation strategies, accordingly, providing retraining to staff as necessary.

· Completes required assessments i.e., safety plans, evacuation, voting?

· Ensures residential, medical, behavioral and other clinical portions of the ISP are completed for services provided or contracted by Ada S. McKinley.

· Completes implementation strategies for outcomes written by ISC within 20 days of receiving parent/guardian approved PCPs and documents implementation strategy approval attempts (3 minimum). • Providing all home staff with initial training on implementation strategies, and additional training as requested by home management. Maintains documentation of training for all direct care staff.

· Develops outcomes that support quality of life and represent what is important to and for the individual served.

· Ensures staff are trained in all required topics, including but not limited to First Aid, CPR, Individual Rights, Incident Management and Reporting, ISP goal documentation, and all required trainings; conducting relevant training as needed

· Coordinates service delivery and case management for services provided or contracted.

· Completes Mental Health Assessment for all individuals served with primary mental health diagnoses (SLOF), including quarterly and six-month reports.

· Attends all psychiatric appointments for individuals on their caseload and ensures required paperwork is complete/provided.

· Supervises waiver authorizations to include identification of waiver services needed as part to the ISP process or changing needs of an individual, assures documentation of waiver services, and secures authorizations prior to expiration.

· Coordinates and collaborates with day program and visits work locations.

· Administrative supervision of health care functions to include coordination of medical appointments, medication administration, medication exchange and health care documentation.

· Assists with management of individual funds in accordance with policies and procedures. • Manages all data collection. • Completes quarterly reports and any other report that is needed based on the person-centered plan. • Providing clear and concise, step-by-step instructions for implementation by direct care staff.

· Providing all home staff with initial training on implementation strategies, and additional training as requested by home management. Maintains documentation of training for all direct care staff. • Attends monthly team meetings for homes where participants on their caseload reside, as requested.

· Attends all required meetings, trainings and court appearances.

Requirements:

· Required Bachelor’s Degree in a human service field or similar area from an accredited college or university

· Must be Qualified Intellectual Disabilities Professional (QIDP) eligible or certified

· Required 5 years of experience working directly with people with Intellectual / Developmental disabilities.

· Required 3 years of progressive management experience.

  • A valid driver’s license, working vehicle, car insurance and less than 3 moving violation within the last 5 years
  • The selected candidate must be 18 + years old

· This position requires the individual possess the ability to coordinate effectively with other team members, other Agency resources and outside services in addressing client needs.

· This position requires the individual possess an understanding of evidence-informed practices, engagement strategies, boundary setting, concurrent documentation, working in a community setting, organizing, motivational interviewing, and family systems.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

While performing the duties of this job, the employee is subject to sitting, walking, and standing for prolonged periods; frequently pushes, pulls, grasps, feels, and occasionally stoops, kneels, crouches, or crawls. The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee is subject to assist and lift up to 50 pounds with varying amounts of assistance on a recurring basis. The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Must have the ability to perform CPR and CPI. Occupational exposure to blood, body fluids, and infectious substances, including communicable diseases, when performing personal care (Category III). Occasional exposure to cleaning agents and disinfectants.

Environmental Conditions: While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts and outside weather conditions. The noise level in the work environment is usually loud.
