Job Summary:
The Community Care Registered Nurse at Huron Valley PACE works in a close collaborative relationship with a Nurse Practitioner and Licensed Social Worker to provide integrative care to frail elders with complex needs. The RN utilizes a systematic approach to practice incorporating clinical assessment, planning, implementation and evaluation of outcomes to pro-actively manage and advocate for PACE participants. The focus of care provided is to enhance functional capacity, encourage autonomy, and assure coordination of care and services.
Qualifications:
- Registered Nurse with current Michigan licensure, BSN preferred
- Must possess a current State of Michigan driver’s license and maintain an acceptable driving record
- BLS for healthcare providers or CPR Certification
