Faith Hospice has a unique opportunity for an LPN to join their Trillium Palliative Services Team. This LPN will work 2 weekdays per week in the palliative services clinic.

Our Trillium Palliative Services team provides relief and comfort to anyone suffering with the symptoms of a life limiting illness - helping relieve pain, shortness of breath, nausea, loss of appetite, fatigue, depression, sleep problems, and treatment of side effects. Palliative care can be administered as needed by our team of certified and specialized palliative providers to help those affected cope physically and emotionally. We can also help guide caregivers and family members through the complex medical system to better understand the care options available.
What Will You Do?
This key team member will:
- Provide nursing services including assisting provider with examinations and procedures, taking and recording vital signs, evaluating test results, etc.
- Address patients' health concerns while building and maintaining trusting relationships with patients and their families
- Coordinate scheduling of appointments to see the provider
- Answer office phones, assist with office duties, and complete required documentation
- Maintain communication with other disciplines and outside service providers to facilitate quality patient care

Education and Experience
- LPN licensure in the State of Michigan
- Physician office experience
- Hospice and/or palliative care experience preferred
