Position Summary:
We are seeking a dedicated and experienced Certified Nursing Assistant (CNA) Team Leader to join our Primecare team. As a Team Leader at Primecare Home Care, you will play a crucial role in coordinating and leading the caregivers to ensure the highest level of patient care. This role requires strong leadership skills, excellent communication and a deep commitment to providing compassionate and quality care. You will collaborate closely with the Respect & Care Center and the Human Resources department to ensure a seamless process for the caregivers.
Responsibilities:
Supervise and lead a team of caregivers, providing guidance, support, and feedback.
Oversee and coordinate the delivery of patient care by the caregivers, ensuring adherence to established protocols and standards.
Collaborate with the Employee Specialist when applicable in initial interviews and assess
candidates; compatibility with our company values and mission.
Provide a smooth transition for new caregivers, offering support and guidance throughout the first 90 days of the onboarding process.
Collaborate with the Staffing Coordinator to match caregivers with open shifts based on
availability, skills, and client requirements.
Collaborate with nursing staff to optimize patient concerns and opportunities.
Partner with the Employee Specialist to ensure credentials are submitted on time and
within compliance.
Regularly audit and review caregiver files and documentation, ensuring compliance with
industry regulations and company policies.
Identify any discrepancies or gaps in caregiver records and take necessary actions to
address them promptly.
Conduct training sessions for new caregivers and existing CNAs, focusing on best
practices, safety protocols, and updated healthcare procedures.
Conduct quarterly caregiver meetings to ensure caregivers are up to date with company
expectations and recognition.
Provide ongoing education to the team to enhance their skills and keep them informed
about the latest developments in healthcare.
Monitor and evaluate the performance of the Caregivers, identifying areas for
improvement and implementing corrective actions as needed.
Complete Annual Performance Reviews of caregivers.
Conduct home visits to ensure that caregivers are adhering to the Plan of Care.
Administrative functions such as documentation of completion of tasks
Communicate important updates, policies, and procedures to the CNA team in a clear and
timely manner.
Required Knowledge, Skills, Abilities and Competences:
Current certification as a Nursing Assistant (CNA).
Energetic, positive, supportive, encouraging and overall pleasant disposition.
Ability to prioritize multiple projects, manage production schedule, and meet deadlines.
Ability to introduce change, implement new programs, and strategies and ability to
maintain program performance.
Strong leadership and interpersonal skills.
Excellent communication and organizational abilities.
Ability to maintain confidentiality when handling and assessing sensitive employee
information; protect privacy related to any employee or candidate information.
Strong proficiency in data analysis tools (e.g., Google Suites and Microsoft Suites).
Must be able to work 2 weekends a month.
Minimum Qualifications:
● Certified Nurse Assistant (CNA)
● Current in person First Aid and CPR certification
● Current Tb Skin test within 30 days.
● Excellent communication skills, both written and verbal.
● Detail-oriented and skilled in auditing and compliance.
● Compassionate, patient, and able to build rapport with caregivers.
● Proficient in Microsoft Office Suite and data entry.
● Ability to work independently and collaboratively within a team environment.
● Willingness to accept additional duties as needed to help department team and other
departments duties as assigned.

Pay: From $45,000.00 per year
