Job Title: Clinical Therapist
Unit: Adult Services
Location: Detroit/Westland
Reports To: Program Manager of Clinical Services
Work Schedule: M-F, 8:30-5pm, 3 days in office, 2 days WFH

POSITION SUMMARY:
The Clinical Therapist will provide mental health therapy services within a primary care-based clinic. This position requires the formulation of an individualized treatment plan which will be used in conjunction with the entire primary care team to develop a holistic care strategy for each client.
EDUCATION AND EXPERIENCE:
- LMSW, LLMSW, or LPC
- 2 years of clinical experience
- We are looking for therapists to serve in our Life Choices program (I/DD), our Older Adult Services program (SMI) and therapist trained to work with both populations.
RESPONSIBILITIES AND DUTIES:
Person Centered Planning includes completion of the person-centered planning document that provides an overview of the person and details his/her hopes, dreams and desires.
Individual Plan of Service includes completion of treatment goals, measurable objectives, assigned staff and applicable treatment codes.
Consent and Enrollment includes completion of required consent and enrollment documentation.
Documentation includes complete, accurate and timely completion of clinical and statistical records of staff activities.
Documents every consumer and collateral contact in compliance with agency policy.
Complete various administrative forms to assure accurate data collection for billing, fee setting, the management information system and other purposes in accordance with agency policy
SKILLS AND ABILITIES:
- Must possess excellent MS Office Skills (Word, Excel, PowerPoint, Outlook)
- Must demonstrate excellent multi-tasking and time management skills
