We have a full-time opening for a Care ManagementClinical Supervisor. The SFHP Care Management department runs manages several programs to meet the needs of SFHP members, including the Complex Case Management (CCM) and Time Limited Coordination (TLC) programs, to support Medi-Cal members with complex psychosocial needs and high utilization of acute health services. Reporting to the Manager of Care Management, you are responsible for the clinical and administrative oversight of a team, consisting of Intake Coordinators, Community Coordinators and a Care Management Nurse with input from a multi-disciplinary team. In addition, the Care Management Supervisor acts as clinical lead for subpopulations served by SFHP and is a member of both the SFHP Care Management and Health Services Management Teams where they are expected to actively participate and represent the needs of their staff and the members served.
WHAT YOU WILL DO:
- Provide supervision to the Intake Coordination team and Care management staff, including licensed (RN/ASW) and non-licensed (Bachelors level) individuals.
- Support in program strategic development; including creation of workflows, desktop procedures, policies and hiring plans, primarily for the Intake Coordination team.
- Provide oversight of Health Risk Assessments (HRA), Health Information Form (HIF)/Member Evaluation Tool (MET) and the intake process for Enhanced Care Management (ECM), Community Supports Programs, and various Population Health Management (PHM) initiatives.
- Assist the Director and Manager of Care Management in messaging and providing a consistent unified vision to all Care Management staff.
- Provide supervision and clinical input and oversight to direct report Care Management staff.
- Provide regular feedback to supervisees on their work performance, focusing on both areas of strength and opportunities for improvement. Provide support and guidance to staff in order to help them achieve and sustain expected productivity indicators.
- Regularly audit cases to ensure staff understanding and implementation of program priorities and principles.
- Identify areas for program improvement and make recommendations to department management.
WHAT YOU WILL BRING:
- Masters degree in Social Worke or Psychology with 2 years minimum clinical experience
- Unrestricted California LCSW or MFT license
- 2 years facilitating individual and group supervision.
- 2 years of work experience with complex populations (homelessness, mental illness, substance use) preferably with Medi-CAL recipients.
- Knowledge and work experience with CalAIM initiatives such as Enhanced Care Management and Community Supports highly desired.
- Bilingual in Spanish, Cantonese or Mandarin preferred but not required.
- Because this position may require member home visits, the incumbent must successfully complete a sex offender registry screen.
- Knowledge of community social service agencies
- Working knowledge and use of Trauma Informed language and population management principles desired
- Motivational skills to inspire and facilitate staff engagement and learning
- Experience in behavior change coaching
- Experience in mentor individual and team growth
