Form of workClinical Navigator Position Summary: Assists patients in understanding their diagnosis, treatment options and the resources available. + Serves as an essential link between patients and all other medical staff + Facilitates appointments for consults and support services within established service standards, including social workers, dietitians and physicians + Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines + Develops patient education programs and tools + Follows patients throughout the care continuum, including inpatient admissions and collaborates with inpatient care management resources + Adheres to established productivity benchmarks; identifies opportunities to reduce the cost of care + Responsible for outreach efforts to establish and maintain positive working relationships with medical staff + Performs other related duties as assigned + Familiar with standard concepts, practices, and procedures within the field + Relies on experience and judgment to plan and accomplish goals + Regular and predictable attendance is an essential job function + Competent to meet age specific needs of the unit assigned + Full Time -Day Shift - 80 Hours Per Pay Period Requirements and Minimum Qualifications: + BSN required; MSN preferred + Graduate of a nationally accredited nursing program + Licensed as a RN by the Idaho Board of Nursing OR licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multi-state privilege to practice" compact with Idaho + Minimum of 3 years of relevant nursing experience required + Certification within specialty area required within one year of meeting qualifications + Must complete KH BLS during orientation period Working Conditions: + Must be able to maintain a standing position + Typical equipment used in a clinical job + Repetitive movements About Kootenai Health Kootenai Health is a highly esteemed healthcare organization serving patients throughout northern Idaho and the Inland Northwest.
