St. Luke’s Health System in Meridian is seeking a Nurse Navigator to join our SL Pediatrics Developmental Clinic team.
The Nurse Navigator will assist with rooming patients, vitals, referral triage, genetic and genesight testing, prior authorizations, medication refills, assess patients' emotional, clinical, financial psychological needs, answer questions during initial consultations, patient education, provides resources, and more.
Responsibilities
The Nurse Navigator offers medical guidance, acts as a support group, and guides patients and their families through the treatment process.
- Serves as a single point of contact for patients undergoing treatment,.
- Serves as a liaison between the patient and their family and the patient's network of care providers.
- Educates patient on diagnosis, treatment options, and prognosis.
- Assists the patient in scheduling appointments, completing paperwork, or securing referrals.
- Facilitates short term and/or long term interventions.
- Possesses comprehensive knowledge of subject matter.
- Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
- Other duties and responsibilities as assigned.
Qualifications
- Education: Bachelors of Nursing Degree
- Experience: 2 years' of RN experience
- Licenses/Certifications: Current RN License in the State of Practice
Preferred Qualifications:
- Pediatrics and/or Case Management Experience
- Strong and effective communicator
