PrimaryOne Health is currently seeking a Clinical Case Worker for our East Broad health center.

Essential Job Responsibilities:*
- Provide high quality case management for clients and their families by assisting them to access medical services, health insurance, Ryan White benefits, services to improve health outcomes, housing stability, employment and income attainment, and other resources.
- Conduct comprehensive psychosocial assessments for people living with HIV/AIDS and seeking services at intake. Complete updated assessments annually and as needed. Clinical Case Workers will assist clients in completing and submitting all necessary documentation related to these assessments.
- Serve as the lead on a multi-disciplinary care team to coordinate care for patients who are living with HIV/AIDS.
- Develop, monitor, and evaluate individual case plans for each assigned client at intake, annually, and as needed thereafter.
- Function as a central and primary access point for financial assistance programs.
- Represent PrimaryOne Health within the community, engaging other service providers and providing education about special needs associated with a client living with HIV/AIDS in the primary care continuum, mental continuum, and other community resources.
- Works collaboratively within a multidisciplinary team.
- Clinical Case Workers are responsible to maintain documentation through applicable software systems.
- Responsible for overseeing compliance with budgets, billing processes of assigned program and compliance with program contracts.
- Other duties or special projects as assigned.

Successful candidates must possess a Bachelor's Degree in Social Work and valid LSW required; MSW preferred. One year of experience in Case Management; integrated care or medical setting experience preferred.*
