As a member of the multidisciplinary team, the activity coordinator will conduct initial assessments and reassessments of all participants. Will develop and implement individualized plan of care based on the participant's assessment. Will complete quarterly progress notes on each participant. Plan and implement all activity programs at the Center and shall actively involve the participants in the planning of the activity programs. Develop and post a monthly calendar of events. Responsible for overall supervision of activity programs. Order supplies, keeps records of activity expenses, make recommendations for supplies, materials, and equipment that will enhance the programs benefit to the participants. Maintain appropriate record keeping and documentation of participants response to activities offered and attended. Conform to all applicable policies of the Center and attend the orientation program offered by the Center. Must have activities certificate, work with population we are serving. Bilingual is Spanish a must
