JOB SUMMARY (overall description of work done by this position)
This position will focus on closing care gaps and panel management in a remote work environment. Tasks include reviewing provider schedules and preparing for upcoming appointments. Charts will be reviewed for open care gaps and patients will be contacted via telephone or myChart to coordinate any pre-appointment or care gap closure needs.

Essential Functions (primary responsibilities of the position in order of importance)

Review Provider schedules to complete a chart review prior to patient appointment

Identify any gaps in care for reviewed patients

3.
Contact patients prior to their appointment to review care gaps and coordinate any pre-appointment requirements

4.
Use dashboards to follow up and coordinate reassessments of specific care gaps (ex: blood pressure checks).

5.
Act as a liaison for Physicians and Advanced Practice Providers and communicate patient needs to Provider via Epic documentation.

6.
Regularly review and keep Provider panels current.

7.
Facilitate completion of forms (ex: W/C, insurance, etc.) by the physicians
