Position Summary
The Palliative Care Nurse works in collaboration with an interdisciplinary team to deliver Palliative Care for patients and families who are dealing with serious chronic illness. The Palliative Nurse will help educate patients and their families with a focus on improving symptom management as well as increasing knowledge of medical options and how they impact quality of life.
- Responsible for utilizing the professional nursing process to provide individualized nursing care to patients across the lifespan in an ambulatory setting. Ambulatory nursing is characterized by the efficient, focused assessment of patients.
- Present a proactive role within the clinic working with Nursing, Pharmacy, Care Coordinators, and other departments to provide Palliative Care services to patients and their families and implements guidelines that support the complex patient throughout the system.
- Care for patients and families in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as patients' transition through care settings.
- Act as a patient and family advocate and assist the patient and family in developing skills so that they can advocate for themselves while preserving and protecting patient and family autonomy, dignity and rights.
- Responsible for the coordination of patient care, screenings, education, triage, and various other tasks within the RN scope of practice.
- Collect, prioritize and synthesize available data to identify unique patient needs.
- Assess cultural and age-specific considerations for appropriate care planning. Accurately document all nursing care and related services in the patient health record.
- Actively participate in ongoing quality improvement activities.
- Provide for safe patient transitions and hand offs through effective communication with patient, family and care providers.
- Provide for patient safety through the use of two patient identifiers, safe medication administration practices, and appropriate hand hygiene.
- Follow current National Patient Safety Goals.
- Maintain current knowledge in nursing practice and participate in ongoing educational related activities.
- Attend educational opportunities to learn specialty care procedures and clinic processes.
- Must be able to provide education to patients via various methods.
- Maintain current nursing licensure as required.
- Maintain compliance with departmental policies, procedures and clinic processes.
- Help ensure compliance with regulatory or accreditation processes.
- Other job duties and responsibilities as assigned to effectively meet the needs of the patients, the department, and the organization as a whole.

Requirements
Education
- Graduate from a Nursing program or Associate's Degree (two year college or technical school) Required, Field of Study: Nursing
- Bachelor's Degree Preferred, Field of Study: Nursing
Experience
- 3-5 years Nursing experience in a clinic setting preferred.
- Palliative care experience preferred.
- Patient education experience preferred.
Computer Skills
- Proficient in basic computer skills.
Certifications & Licenses
- BLS
- Active Nursing License (RN) eligible to practice in the State of Wisconsin
