Clinical Care CoordinatorResponsibilities

• Provides care as defined within the scope of practice
• Makes the initial evaluation visit and regularly reevaluates the home care clients nursing needs
• Prepares the clients initial assessments and plan of care
• Completes regular supervisory visits and reassessments on clients and care providers
• Assists with in-service and training programs
• Performs skilled services such as injections, wound care, medication set up, cath care, etc.

Clinical Care CoordinatorRequirements

• Current unrestricted RN license
• Reliable car/full coverage auto insurance
• Clean background and criminal record
• Understands regulatory compliance with state and federal regulations
• Adhere to HIPAA and maintain client confidentiality
• Demonstrated ability to supervise and direct staff.
• Possess exceptional comprehensive nursing assessment skills
• Comfortable with various computer systems and electronic medical records, data entry, email
• Comply with accepted professional standards of practice.
• Treat clients, staff and the public with courtesy, respect and presents a positive public image.
