Overview:
To provide ongoing follow up of all emergency room patients that has abnormal x-rays, EKGS, and lab work. Provide follow-up information to patient in order to facilitate a continuum of care. Collaborates with the appropriate emergency room physician to formulate then initiate a specific plan of care for that individual while maintaining accurate and concise documentation that culminates in a positive outcome. The Care Navigator is responsible for assessing patient individual needs for physical, well being of the patient. The Care Navigator connects the patient/family/support system with appropriate resources, services, and referral sources. The Care Navigator facilitates effective transition planning as part of the Mercy Care Management Model.

Qualifications:
- Education: Associate degree in nursing.
- Licensure: Current Arkansas license as a registered nurse. Current unrestricted AR drivers license.
- Experience: Two years recent applicable nursing experience (senior adults and geriatric) with outstanding working knowledge of Medicare, Medicaid and other geriatric resources (e.g., nursing homes, hospice, etc.). Excellent computer skills; electronic health record skills preferred.
- Certifications: Current Basic Life Support. Case Management (or ability to become certified within 36 months of accepting employment)
- Other: Excellent problem solving, interpersonal and team player skills. Ability to establish and maintain outstanding working relationships with nursing homes and other referral sources. Projects and promotes good personal and public relations. Ability to balance multiple tasks at once and prioritize effectively and efficiently. Ability to travel by car; valid AR drivers license.
