Job Description Purpose of Position: In this role, the successful candidate will work closely with physicians, hospital staff, and administration to ensure the availability of complete, accurate, and timely medical records.
Perform quality reviews and assess regulatory compliance with the completeness of data and information, timeliness, accuracy, and authentication requirements following Joint Commission, CMS, other federal and state agencies, as well as Hospital Medical Staff Bylaws. Maintains HIM integrity through knowledge of medical record content, medical terminology and abbreviations, abstracting and analysis techniques, concurrent medical record review, and chart correction. Knowledge of state and federal HIPAA laws on the privacy, security and confidentiality of protected health information (PHI).
Participates in intra-departmental Quality Assurance and Quality Improvement efforts. Duties and Responsibilities: Lead chart completion staff in conducting daily concurrent and final analysis of inpatient and/or outpatient medical records for accuracy, errors, and deficiencies; assign (in Epic) to appropriate providers for completion, correction, and/or signature. Establish collaborative partnerships with Data Integrity, ROI, Medical Staff, and other stakeholders to ensure timely availability of accurate health information documentation, prompt error correction, and access to records for continuity of care and coding/reimbursement activities.
Liaise and work closely with other teams (EMPI, IT, Registration, Finance, etc.) to correct patient data errors, resolution of related issues, review, create, and improve workflows and reporting. Generate daily, weekly, and end-of-month reports to track productivity and statistically relevant data. Monitor performance of transcription service including resolution of issues preventing the interface of documents into the electronic medical record (Epic).
Manage and monitor HIM Work Queues for timely completion of accounts. Provide orientation and training to residents, house staff, and new attending physicians regarding record completion requirements. Coordinate physician separation of service with the Chart Completion team, Human Resources, Medical Staff Office and other departments, as applicable.
Assist with medical records audits/requests including those originating from Risk Management, Quality Management, Finance and other departments. Attend staff, Leadership and other meetings, as directed. Perform other duties as assigned.
Minimum Qualifications A four-year High School Diploma or its educational equivalent; and Six (6) months of full-time satisfactory clerical experience and/or experience interacting with the public, such as processing sales; taking, processing, and delivering orders; obtaining and dispensing information; listening to and resolving customers concerns, problems, and complaints; and/or performing related customer service activities. Skills Requirement: Keyboard familiarity with the ability to type at a minimum of 100 key strokes (20 words) per minute.
