Additionally, we are pleased to offer a one-time bonus of $7500 after one year of service, contingent upon successful performance evaluations and meeting performance expectations. Population/Needs:

Bring your passion and expertise to life in a dynamic role as a Licensed Clinical Social Worker (LCSW) with a proven experience of over three years specializing in providing dedicated care to children and adolescents. Showcase your documented understanding of child and adolescent development, leveraging a wealth of knowledge in behavioral interventions and therapies specific to address the unique needs of this demographic. The ideal candidate should possess a understanding of clinical best practices but will also be familiar with the local geographic area. This geographic insight serves as a powerful tool, enhancing your ability to collaborate seamlessly with other agencies and community resources, thereby creating a network of support for those you serve.

Principal Responsibilities:

  1. Work at a Federally Qualified Health Center as a part of a multidisciplinary team to ensure that the patient’s clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultation, and individual meetings with staff to ensure quality of care.
  2. Conduct functional and strength-based bio psychosocial assessments for children and adolescents including comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs.
  3. Develop patient care plans for children and adolescents that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
  4. Provide psychotherapy, counseling, medical case management and psychoeducation utilizing individual, family, and group modalities.
  5. Employ Evidenced Based brief oriented therapies for children and adolescents including sand and play therapy, knowledge of family systems, experience with family therapy and teaching parenting skills.
  6. Working with children and their families that include social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, and relaxation as well as access to necessary resources with the goal of improved functioning and positive health outcomes.
  7. Utilize both short-term and intensive longer-term interventions, as approved by funding source that target trauma, mental health symptoms, increase psychosocial functioning and reduction of substance use.
  8. Interventions are client centered; strength based, and identified based on a comprehensive treatment plan that is developed, after completing a thorough biopsychosocial assessment.
  9. Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches that incorporate behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
  10. Participate in Multi-Disciplinary Team meetings are held weekly in the clinics.
  11. Provide consultation to and coordinate care for patients needing specialty mental health and/or substance use disorder services.
  12. Advocate for patients within organizations and community to ensure access to appropriate care.
  13. Be available to primary medical care staff during working hours for crisis intervention in the event of a psychiatric emergency and assist staff in the event of any other kind of emergency.
  14. As Officer of the Day be available for urgent consultation and clinical sessions with patients who have urgent needs and provide consultation to providers who may have urgent needs and consultation and to clients who present with immediate psychosocial needs.
  15. Complete documentation for other special programs such as Ryan White Mental Health or others.
  16. Be available as assigned to provide after-hours emergency psychiatric assistance.
  17. Provide phone consultation as needed.
  18. Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation.
  19. Productivity goal is to average 8-10 patients per day.
  20. Participate in structured and “as-needed” case conferences.
  21. Participate in behavioral health quality assurance/improvement activities to maintain and strengthen standards of care, in accordance with the organization CQI Plan
  22. Provide Clinical Supervision for ASW’s in accordance with BBS Guidelines.
  23. Support client involvement in planning, implementing, and evaluating services.
  24. Complete data collection forms and prepare reports as required by the program.
  25. Other duties as assigned.

Requirements:

  1. Master’s degree in social work, social welfare from an accredited college or university.
  2. LCSW with current California BBS license
  3. 3 plus years working with Children and Adolescents
  4. Experience working on an Interdisciplinary Team
  5. Demonstrated understanding of the impact of trauma on behavioral and medical conditions
  6. Knowledgeable of evidence-based treatments (sand and play therapy, DBT, CBT, Seeking Safety, Motivational Interviewing)
  7. Knowledge of principles, procedures, methods and techniques of mental health emergency procedures such as: 5150, CPS reporting, APS reporting, and Tarasoff reporting
  8. Experience working with substance abuse disorders preferred.
  9. Experience working in a medical setting preferred.
  10. Bilingual (English/Spanish) ability needed.

Note:

If you become a JWCH provider, you may be able to participate in Loan Repayment Program. A majority of our clinics are NHSC approved sites. The National Health Service Corp may be accepting applications Loan Repayment Program awards. The program is open to licensed primary care medical, dental, and mental and behavior health providers who are employed or have accepted an offer of employment at an NHSC-approved site. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II). You will need to contact National Health Service Corp to apply and receive more information. Upon acceptance, you are encouraged to apply to National Health Service Corp for Loan Repayment Program.
