Reporting to the Assistant Program Director/Director of Social Services, the Case Work Supervisor is responsible for the day-to-day supervision of the Case Managers and Housing Specialist. The Case Work Supervisor ensures the Case/Housing Management team engage clients in the provision of services and are collaborating with the clinical team in the delivery of comprehensive holistic services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:- Ensures staff meet with all new clients in order to assess client needs, complete all intake forms and develop initial service and housing plans which will serve to guide the services they will receive by program staff and external organizations.
- Ensure clients are referred for appropriate entitlements and benefit services and receive assistance in obtaining required documentation in order to apply for benefits, housing and other services.
- Ensures housing staff is referring clients for supportive housing and are assisting clients with the application process.
- Collaborates with operations team and coordinates room inspections.
- Ensures participation in team meetings and on-going collaboration with the clinical team to ensure the on-going assessment of client needs and identified services are effectively coordinated.
- Utilizing the DHS CARES System Supervisory Function and audits charts to ensure compliance with OTDA and DHS documentation requirements and monitor the delivery of quality services.
- Provides staff training, coaching and on-going supervision on trauma informed care, evidence-based practices and care coordination.
- Provides crisis intervention support to staff and responds to incidents. Ensures the timely and accurate completion of incident reports.
- Participates in the development and implementation of staff training and Quality Improvement projects.
- Supports client participation in workshops, support groups and therapeutic activities by identifying clients who can benefit from these services and minimize barriers which impede in client participation.
- Participates in team meetings, case conferences, staff trainings, etc.
- Ensures the accurate and timely submission of data/reports. Reviews data/reports and dashboards to ensure staff are meeting their assigned weekly targets. Develops strategies with staff to ensure consistency in performance.
- Serve as a super user in the DHS CARES System to assist the social service staff in trouble-shooting problems in the system.
- Interviews candidates and makes hiring recommendations. Provides on-going performance deed back and reviews in accordance with agency policies. Collaborates with DSS/APD and HR in addressing staff performance issues.
- Perform other duties as assigned.
- Bachelor’s degree in Social Work or related field required.
- Minimum of one to two years’ experience working in a social services setting with individuals who are experiencing mental illness, substance abuse and homelessness.
- Excellent interpersonal and communication skills, both written and verbal.
- Solid computer skills, including Microsoft Office and Outlook.
- Knowledge of CARES preferred.
- Bilingual in Spanish preferred.
- Lifting and moving objects weighing up to 30 pounds several times a week.
- Climatic issues of temperature variance and ventilation association with resident facility impact delivery of services.
