Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Compensation Range: $60,000 - $65,000 annually commensurate with experience
Overview: 
In collaboration between Housing Works Inc. and the Mayor's Office of Criminal Justice (MOCJ), the Emergency and Transitional Housing (ETH) Program will provide individuals involved in the criminal legal system experiencing housing insecurity, or chronic homelessness, with transitional housing placement and reentry support services with the goal of providing assistance with more permanent housing solutions. This housing program operates utilizing hotels across NYC boroughs and offers medical, behavior health, reentry support services, connection to additional health and wellness services and housing placement assistance to residents enrolled to promote positive reintegration to the five borough communities, reduce recidivism and build stronger, safer communities. Reporting to the Assistant Director of Housing Support Services the Case Management Supervisor will be responsible for providing supportive supervision to the Case Managers assigned to the ETH site, conduct quality assurance and fidelity measures to ensure services are provided in accordance with contract compliance, participate in quality improvement projects, data reporting and potentially maintaining a caseload of high-need clients.
Responsibilities:

  • Provide supportive supervision and guidance to the 3-4 Case Managers assigned to the Transitional Housing site.
  • Provide day-to-day leadership for support services provided at assigned location.
  • Conduct quality assurance and fidelity measures to ensure program data outcomes align with contract deliverables.
  • Thoroughly review client charts and ensure documentation meets Quality Assurance Standards.
  • Lead care coordination efforts at the assigned location.
  • Facilitate interdisciplinary case conference meetings.
  • Provide overall leadership support for case management and care coordination services.
  • Participate in quality improvement projects.
  • Identify, or facilitate, staff trainings to promote professional development and the enhancement of technical and clinical skills.
  • Assist with integrating the Transitional Housing Program services with agency initiatives.
  • Lead onboarding and training for new staff members.
  • Facilitate and coordinate team meetings and case conferences.
  • Maintain a caseload of high-need clients, if necessary.
  • Promotes Housing Works' core values in day to day activities.
  • Performs other tasks as assigned by supervisor.
  • Provide culturally competent services in accordance to Housing Works policies and practices.
  • Promotes Housing Works core values consistently.
  • Engage in advocacy efforts in accordance with Housing Works' and the Justice Initiatives Department requirements.
  • Ability to work with, and guide supervisees to work with, clients with diverse clients with varying levels of service needs.
  • Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.

Other functions that may be required with this position include:

  • Attend and participate staff meetings and trainings as scheduled.
  • Responsiveness to emails, texts and phone calls.
Minimum Requirements 
  • High School Diploma required.
  • Bachelor's Degree preferred in social services, psychology, social work or other related field.

The following are qualifications and requirements of the position:

  • Minimum 3 years of experience providing case management or direct client service.
  • Minimum of 1 year of supervisory experience.
  • Experience with criminal justice involved population, as well as substance use and mental health issues.
  • Strong written documentation and communication skills
  • Good problem-solving skills and ability to work autonomously
  • Microsoft Office Suite, particularly Excel, required
  • Experience utilizing EMR systems.

The following additional competencies are viewed as important to success in this position:

  • Preferably bi-lingual in Spanish.
  • Excellent written and oral communication skills.
  • Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.
  • Ability to work independently and collaboratively with the team.
  • Dependability and reliability to arrive and depart timely for scheduled shift.
  • Ability to frequently travel within the boroughs via public transportation.
  • Flexibility to work evening hours as needed to meet the client service needs.
