Job Description
BronxWorks is hiring one (1) Case Management Supervisor for the Homebase Program.
RESPONSIBILITIES
- Review client intakes and determine client eligibility for Homebase services.
- Review cases, to ensure that they meet ESG/EAF eligibility requirements.
- Ensure that ineligible clients are referred to appropriate alternative services.
- Ensure that client records are in compliance with ESG/EAF guidelines.
- Review HRA guidelines with staff to ensure that case managers understand ESG/EAF requirements.
- Review HRA database frequently to ensure that Case Management staff is complying with HRA quality assurance requirements.
- Conduct weekly supervision with staff and complete supervision form.
- Complete individual monthly reports for line staff Review Financial Assistance Requests Assess with case managers the risk of clients entering shelter system Review and sign all client service plans.
- Review and sign documentation in client files, where supervisory signature is required Conduct Quality Assurance reviews to ensure cases and case records (paper and electronic) meet ESG/EAF standards.
- With Program Director, create disciplinary plan of action for staff not meeting their job requirements
QUALIFICATIONS
- Master's Degree required.
- Prior supervisory experience working in a social service agency preferred
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment
PHYSICAL REQUIREMENTS
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
