Description
Alert JFS is looking to hire an experienced, compassionate Case Manager (Social Worker/MSW) to assist individuals and families in maintaining and/or improving their independence, level of functioning and quality of life by providing for and/or arranging for an array of social services.
Case Manager (Social Worker/MSW) Senior Population Essential Responsibilities:
- Work effectively with partner agencies as well as other organizations that provide assistance to Holocaust Survivors
- Participate in weekly supervision with Case Managers and weekly agency staff meetings
- Perform bio-psychosocial assessments including analysis of and independent judgment in the areas of physical functioning, psychological/emotional functioning, mental status, activities of daily living, instrumental activities of daily living, social functioning, nutrition, and environment. These assessments help determine the client's needs and are performed in client's home setting
- At one's discretion, develop service plans based on assessment to meet client's needs and to establish a working relationship
- Independently refer for and coordinate needed services internally and externally
- Advocate to meet client's needs; consult and collaborate with other professionals and service providers to assure comprehensive care
- Counsel and support client's families to maximize their well-being
- Monitor the ongoing needs of clients through telephone and face-to-face interaction to evaluate the treatment process and modify if necessary
- Document all activity in electronic record keeping system on a timely basis and maintain case records to meet quality standards
- Analyze and respond to emergencies/crises to maintain quality care
- Maintain caseload size as required by the agency to meet professional goals
- Facilitate support/therapeutic groups to encourage group self-help if requested
- Respond to all communications in a timely manner for care continuity
- Coordinate all internal referrals to other agency programs
- Conduct initial telephone inquiries and complete all appropriate paperwork timely
- Ensure compliance with grants and monitor clients' service hours to maintain appropriate allocations
- Build and manage relationship with Home Health Care vendors
Requirements
- Graduate degree from an accredited School of Social Work or other Behavioral Mental Health program.
- 3-5 years' experience in a similar role
- Experience with geriatric and special populations
- Knowledge of MS Office Suite (Word, Excel) and familiarity with ERM systems
- A valid Florida driver's license is required and must have a good driving record
- Proof of automobile insurance
- Strong communication and interpersonal skills, flexibility, and ability to function independently
- Possess an understanding of and compatibility with Jewish values and traditions
