Corporate Talent Resources LLC is seeking an experienced Case Manager in Houston Texas for a FT permanent on-site position.
Must Haves To Be Considered
- BSN or MSW
- Texas Active RN or MSW License, LCSW preferred
- Minimum 3 years of nursing or social work experience, acute hospital-based preferred, or
- Minimum 3 years of experience in a comparable clinical setting (i.e., ambulatory surgery center, infusion/dialysis clinic, Federally Qualified Health Clinic (FQHC), skilled nursing facility, or wound clinic)
- Certification in Case Management is strongly preferred or ability to obtain within 2 years of hire in the position
- Experience in utilization management, case management, discharge planning, or other cost/quality management programs preferred.
- Current working knowledge of discharge planning, utilization management, case management, performance improvement, disease or population management and managed care reimbursement.
Overview of Position
The role of the Case Manager is to provide support to physicians, primary medical homes, and interdisciplinary teams by facilitating patient care. This involves aiming to improve clinical outcomes and patient satisfaction while also managing the cost of care and delivering timely and accurate information to payors. In addition to coordinating resource utilization management, care facilitation, and discharge planning, Case Managers drive change by identifying areas for performance improvement, such as workflow, education, and enhancing patient satisfaction. They are responsible for coordinating self-management support and maintaining disease registry activity. With accountability for a designated patient caseload, Case Managers effectively plan to meet patient needs across the care continuum, provide family support, manage length of stay, and optimize resource utilization.
