JOB TITLE: Case Manager - Illinois Welcoming Center
FLSA STATUS: Full-Time Exempt
REPORTS TO: Family Services Program Manager
Brighton Park Neighborhood Council seeks a Case Manager to provide case management services to immigrant and immigrant families in the Brighton Park Community. Brighton Park Neighborhood Council (BPNC) is a non-profit organization located on the Southwest side of Chicago.
The Case Manager reports to the Family Services Program Manager and works directly with clients to provide comprehensive and culturally sensitive case management and service coordination to immigrants, refugees, and asylees. The Case Manager will assist clients in developing short-term and long-term goals to address the individual or family’s needs. The Case Manager will support clients in connecting to resources related to health, housing, food, language, legal services, civic engagement, and other needs. Services will be provided at the BPNC office, partnering agencies, or at shelters. This position is full-time, 40 hours per week.
This is a full-time temporary position that will run until June 2024, with the possibility of the position being extended.
Responsibilities
- Provide comprehensive and culturally sensitive case management services to all clients, including intake assessment, benefit assessment, financial goal setting, educational goal setting, money management, and financial literacy training.
- Weekly case plan development, progress monitoring, advocacy, and referrals.
- Assist 100 households in a year.
- Meet with clients weekly or bi-weekly. For the first three months of the client’s participation, 80% of follow-up meetings should be held in person rather than over the phone.
- Connect participants with needed and available community resources—follow-up with clients and agencies as appropriate to document the use/success of the referral.
- Input accurate and complete data for all contacts with clients into the agency database.
- Provide all required documentation on time.
- Participate in regular staff meetings, staff training, supervisory sessions, and other duties as assigned by the supervisor.
- Flexibility to work evening hours required and occasional weekend hours, when needed.
- Adhere to agency policy, procedures, and the professional code of ethics.
- Other duties as assigned.
Required Qualifications
- Able to work 2 days a week 11am-7pm and 3 days a week 9am-5pm
- Bilingual Spanish/English required
- Drivers License and and reliable transportation
- Bachelor’s degree in Social Work or related field (preferred)
- Knowledge of community resources and counseling/social work practices with high-risk populations.
- Good documentation, skills, and verbal and written communication skills. Ability to establish rapport.
- Ability to motivate others towards achieving goals.
- Ability to work independently with a strong sense of focus, task-oriented, non-judgmental, clear sense of boundaries.
- Ability to work in various settings with culturally diverse families and communities and the ability to be culturally sensitive and appropriate.
- Ability to pass a federal background check before starting the position.
- Ability to complete Mandated Reporter Certification.
- Must have proficient computer skills.
