Job Description
JOB PURPOSE:
Conducting initial assessments and reassessments for members/clients for the elderly and disabled waiver program.
MAIN RESPONSIBILITIES
1. Completes initial and annual assessments in client/member’s home and determines member ability to complete IADLs and ADLs.
2. Completes modified assessments as needed for transfers, hospital and nursing home discharges, and increased or added services.
3. Assures members have services to remain in their community and fulfills the fiduciary obligation to meet members’ needs based on guidelines.
4. Review medications for appropriateness. Makes note of any new medications, dosages, and discontinued medications.
5. Documents assessment entries into online system within guidelines established by the
agency and/or regulatory groups. Requests medical and hospital records as needed to assist with determining eligibility.
6. Complies with company compliance program.
7. Follow established safety procedures when performing tasks and/or working with equipment.
