Job Description: The Case Manager/Intake Worker is responsible for application intake and review for all agency programs available in Bradford and Union County. The individual is required to assist low-income residents within their community with services and information needed to promote self-sufficiency, alleviate crisis and assist with their objectives and goals. The Case Manager/Intake Worker is responsible for maintaining all client records and ensuring that all completed files adhere to program guidelines, as well as maintaining case notes and performing follow-ups. Outreach efforts, reporting, community surveys, as needed, to assess the outcome of agency efforts to eliminate crisis and poverty.
Role and Responsibilities:
- Conduct face-to-face interviews with clients seeking program assistance, process online applications for assistance, collect data and documentation to determine program eligibility.
- Review client documents to ensure program guidelines are met.
- Submit files, reports and documents to the Administrative Office as required.
- Effectively and clearly exhibit professional written and verbal communication skill at all times.
- Answer phones, greets clients and others upon arrival and provide general information on programs.
- Implement and maintain good communications and working relationship with other community resource agencies.
- Respond to program inquiries and refer clients to community resources if needs cannot be met through agency programs.
- Maintain confidential records and information pertaining to clients.
- Perform other duties as assigned.
Qualifications, Skills & Ability:
- Demonstrates excellent customer service skills and the ability to communicate with others effectively listen closely and convey information clearly.
- Must have basic mathematical skills to calculate annual household income.
- Strong written and verbal communications skills.
- Must be detailed oriented, organized and have basic knowledge of establishing and maintaining a filing system.
- Must be able to multi-task, and work in a fast-paced environment.
- Proficiency in Microsoft Office programs.
- Must have the ability to type minimum of 25cwm.
- Demonstrates working knowledge of standard office equipment, including the use of calculators/adding machines.
- Self-motivated and have the ability to work toward objectives with minimal supervision.
