Description
This is professional work providing a full range of Case Management services in the field of drug and alcohol treatment. The employee will screen and assess their clients' needs to determine the level of care required based on placement guidelines. They will be responsible for coordinating access to treatment services and developing, and implementing a comprehensive service plan to include Case Management follow-up.
The employee will also monitor the treatment progress of their clients, write reports, and advocate for clients with other agencies or organizations to maximize clients' benefits. Work is assigned by the Case Management Supervisor in accordance with the objectives of SCA's program and needs. Every client's case is reviewed with the Case Management Supervisor to ensure work is completed in its entirety, and in compliance with the SCA's policies and procedures.
Essential Duties and Responsibilities
  • In accordance with DDAP established guidelines, will conduct telephone screening interviews with clients to determine the need for emergent care, will assess clients with standard placement criteria, will work with provider network to ensure placement of client, and/or will orient, direct and refer callers as appropriate.
  • Will adhere to the roles and responsibilities delineated for Single County Authority (SCA) staff when working with a Health Choices (Medicaid) member who may be receiving clinical and Case Management services through the SCA.
  • Will strictly adhere to all State and Federal confidentiality regulations in releasing or obtaining client information.
  • Will conduct face-to-face interviews with consumers to obtain information needed to provide drug and alcohol level of care recommendations using current approved placement criteria.
  • Will provide documentation of services provided as appropriate for each consumer including necessary paperwork, case notes, updates, ancillary service referrals and/or correspondence generated.
  • Will provide documentation of the consumer's liability data, including verification of income and fee collection as outlined in current policies and procedures.
  • Develops rapport with clients in order to facilitate and follow client through the entire treatment episode while also working with client to assist in resource coordination for ancillary non-treatment needs.
  • Works with providers in ensuring authorizations as well as all appropriate paperwork are in place to maintain the client through the treatment process.
  • Serves as a client's advocate to assist in coordinating inter-professional care with various other community stakeholders.
  • Keeps up to date progress notes on clients in charts on the client progress throughout their treatment stay.
  • Coordinate with the Case Management Supervisor necessary funding based on availability, ensuring changes are addressed in a timely manner. Fax all approved authorizations to facilities to secure/maintain funding.
  • Enter all required forms in the DDAP statewide data system.
  • Will perform all other related work as required.

Required Knowledge, Skills and Abilities
  • Knowledge of addiction as a disease and its treatment.
  • Knowledge of current social, economic, and health problems and the available resources.
  • Knowledge of the treatment facilities and human services agencies providing services to the alcohol and drug clients.
  • Knowledge of the assessment practices.
  • Ability to establish and maintain an effective working relationship with clients from various cultures and social economic backgrounds.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to interpret regulations, policies and procedures and apply them accordingly.
  • Ability to work with persons who have physical, mental, or emotional disabilities, or who are economically disadvantaged, or involved with the criminal justice system.
  • Ability to plan and organize work, prepare records, and reports, set priorities, and maintain a caseload in an effective and timely manner with minimal supervision.
  • Ability to establish and maintain effective working relationships with other members of the staff, outside agencies, and institutions, and the general public.
  • Ability to advocate effectively on behalf of the client.
  • Knowledge of social casework principles and methods.

ELIGIBILITY -ALL CANDIDATE{S):
1.) Meet the minimum experience and training (MET'S) required for this position:
  • One year as a Drug and Alcohol Case Management Specialist Trainee; OR
  • A bachelor's degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; and one year direct service experience performing counseling or Case Management work in alcohol or other drug treatment setting; OR
  • A bachelor's degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; and one year direct service experience performing counseling or Case Management work in alcohol or other drug treatment setting; OR
  • An equivalent combination of experience and training.

ELIGIBILITY - COMPETITIVE PROMOTION WITHOUT EXAMINATION ONLY: CLASS RESTRICTIONS:
1.) Have held regular civil service status in the following classifications:
  • Drug and Alcohol Case Management Specialist Trainee
  • Drug and Alcohol Treatment Specialist Trainee
  • Drug and Alcohol Prevention Specialist Trainee

SELECTION CRITERIA:
1.) Meet the minimum experience and training required for the job.
2.) Meritorious Service, defined as:
  • The absence of any discipline above the level of written reprimand during the 12 months preceding the closing date on the posting.
  • The last due overall regular or probationary performance evaluation was higher than unsatisfactory.

3.) Seniority, Defines: A minimum of twelve months in the next lower class(es) by the closing date of this posting 03/07/2024.

I. Medical Insurance

All full-time employees and part-time employees working more than 1,000 hours per year qualify for Luzerne County retirement benefits, in which employees must contribute 5% of their earnings, and may choose to add to an additional voluntary contribution, not to exceed an additional 10%.
