Summary:
Case coordination assists individuals to achieve physical and mental wellness through the planning of health and social services. This includes but is not limited to advocacy, assessment, planning, and education. The Case Coordinator along with the care team will ensure that the individual is established with services in a coordinated, effective, and efficient manner. The Case Coordinator will offer support as well as monitor and track an individual's progress throughout the course of his/her treatment.
Responsibilities:
- Ensures clients have access to full continuum of traditional and non-traditional services by assessing client needs.
- Monitors and tracks the progress of clients and facilitates team meetings to ensure needs are addressed, escalates and assesses as necessary.
- As part of the Recovery Support Services team, creates, facilitates, and monitors individualized care plans.
- Uses assessment data to drive engagement efforts of the Recovery Specialists
- Works with Nurse Navigator(s) to provide additional support for clients with complex physical health needs/concerns.
- Can provide level of care assessment if needed
- Works closely with the marketing team to grow relationships with community based providers.
- Other duties as assigned
Required Qualifications
Education, Licensure, & Experience
- A master’s degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in a human services field) or other related field.
- A bachelor’s degree with the same education requirements and 1 year of clinical experience.
- An associate’s degree with the same education requirements and 2 years of clinical experience.
- Full certification as an addictions counselor by a statewide certification body, which is a member of a National certification body or certification by another state government’s substance abuse counseling certification board.
- Facilitating process group therapy, individual therapy, and family therapy. Drug and Alcohol/Mental Health experience. Treatment planning experience.
