Job Summary
This is a community-level intervention position that participates in activities to increase awareness of HIV/STI among the general public and access to HIV prevention efforts for those at highest risk. In this role, you will engage in outreach and recruitment strategies for testing and work under the supervision of Senior Program Coordinator to ensure the delivery of program services in accordance with the funded goals and objectives. To be successful in this role, the selected candidate must have some knowledge of HIV/STIs and knowledge of available HIV prevention services and resources in the community. Additionally, must possess the ability to communicate with culturally diverse populations on very sensitive topics, maintain professionalism, and adhere to strict confidentiality with sensitive information. The selected candidate must have reliable transportation, be flexible and able to work some evenings and weekends (as scheduled)
Experience and Education
Minimum Requirements
- Graduation from High school or GED equivalent and no experience required.
- Accomplishes initial intake interviews with patients or their families; explains clinic or hospital procedures; answers patients\' questions concerning clinic or hospital routines; set up schedules for clinic visits as necessary.
- Identifies immediate social, financial, educational, or other family problems, referring those requiring professional attention to other members of social work staff; assists in making patient referrals, following up to determine whether patients\' needs have been resolved.
- Provides initial counseling to patients with respect to certain selected routine problem areas, such as budget management, school absence, parent-child problems, health maintenance, and use of other educational or welfare resources.
- Follows-up patients through home visits, correspondence, or telephone concerning missed clinic appointments, particularly where there is high medical risk involved; assists patients in rescheduling and keeping new appointments.
- Completes patient forms and records; prepares chronological notes on each assigned patient; completes daily and weekly activity and travel reports.
