Social Work Case Aide
Full Time
POSITION OVERVIEW
Case Aides in the Social Services Department are vital team members, supporting and documenting actions and activities with our client families at varying levels of care. A Case Aide must have excellent communication and organizational skills, can effectively and accurately process paperwork and sort files for direct service reference.
Case Aides provide direct ancillary support to our case management teams using sound professional judgment and best practices to ensure the swift provision of services to children and youth living with special medical needs and their families.
Case Aides assist case managers by maintaining confidential information and entering it into various data systems, coordinating partnership visits, and in addition, provide full administrative support as needed either on campus or offsite in 6 counties.
Case Aides provide office support to the case management team within the Social Services Department and liaise with the Client Experience Manager (CEM), other agency staff and external resources staff as necessary.
Responsibilities:
Provide effective, efficient, and courteous interaction between program participants, Case Management Teams, program resources and others. S/he/(they) will oversee quality control practices and ensure the highest level of accuracy in all details relating to participants, their files, and program requirements. Performs other related duties as required.
Essential Functions:
· Complete calls with families and/or resources to gather needed information used for the completion of a service plan or scheduling appointments when required.
· Provides support to case managers, maintaining case files, updating information, interviewing clients, collecting relevant information, and reviewing treatment progress as required.
· Helps clients fill out required paperwork, processing the forms, and make recommendations to the case manager for additional service agencies.
· Assists case managers by communicating regularly with clients’ family members or other caregivers to ensure that they have the information they need about a client’s status
· When necessary, will assist case managers in tracking client progress by keeping detailed records of each client’s condition and treatment plan.
· Coordinating with other staff members in various departments within the organization to ensure that clients receive quality care as requested by the case manager.
· Coordinating with physicians, therapists, nurses, and other medical professionals as necessary to schedule appointments and monitor client’s progress in care.
· In some cases, may assist the case manager by following on referrals of clients/youth to other agencies for additional assistance in cases where the organization does not provide the service needed.
· Assist department staff with transporting donated/purchased goods from other agency providers and delivering to client families
· May help clients find employment or other means of self-sufficiency by providing support and guidance throughout the process under the supervision of the case manager.
· Assist with survey administration and notifying parent of special events.
· Other duties as assigned.
Measurable Deliverables:
· Meet all deadlines as required by program services and other agency partners
· Meet with case managers regularly to discuss clients concerns and determine next steps for each
· Participate in scheduled meetings and team decisions and operations.
· Participate in workshops, seminars, education programs and activities that promote professional growth and development
· Compile, prepare, submit, and maintain accurate records, files, forms, statistics, and additional information in accordance with agency policies, licensing and/or contract requirements
· Work evenings, weekends and holidays as needed or requested by department manager and program staff
· Maintain confidentiality in all areas of client and program operations as required and necessary
· Maintain professional and ethical standards of conduct outlined in Bryan’s House employee handbook including demonstrating respect for agency staff, children, and community members and always complying with required dress code.
